The successful candidate will contribute to delivering a high-quality palliative care service across both hospital and community settings, supporting the prevention, diagnosis and management of illness while ensuring patients and their families receive compassionate, patient-centred care. You will work as part of a multidisciplinary team, collaborating closely with consultants, nursing staff and allied health professionals. The role includes participation in a 24-hour rota, with consultant support available on-site during working hours and off-site out of hours. Main duties of the job * Deliver a comprehensive palliative care service across hospital and community settings. * Undertake acute patient assessment, diagnosis and treatment in palliative care. * Participate in a 24-hour emergency/on-call rota, supporting service delivery in and out of hours. * Work collaboratively within a multidisciplinary team to provide safe, effective and coordinated patient care. * Communicate effectively with patients, families and carers, ensuring they are informed and supported throughout care. * Support the development and improvement of the palliative care service, contributing to innovation and best practice. * Participate in teaching, training, audit and research, contributing to service quality and professional learning. * Promote evidence-based clinical practice and maintain high professional standards in line with national guidance. * Provide leadership by supporting and supervising other members of the clinical team. * Communicate effectively with patients, families and carers, ensuring they are informed and supported throughout care. * Support the development and improvement of the palliative care service, contributing to innovation and best practice. * Participate in teaching, training, audit and research, contributing to service quality and professional learning. * Promote evidence-based clinical practice and maintain high professional standards in line with national guidance. * Provide leadership by supporting and supervising other members of the clinical team.
