Job summary
Poole Community Mental Health Team, Older Persons are recruiting for a full time Band 6 Community Mental Health Nurse. This is an exciting opportunity to join our well established integrated team. If you are an RMN with experience of working with people experiencing a severe mental illness and/or organic mental illness, whether this be inpatient or community based we would love to hear from you.

The service is a specialist team providing assessment, treatment and support for individuals suffering from organic and functional mental health illness. Experience of working with older adults with organic / functional mental illness in an inpatient or community setting would be an advantage. The ideal candidate will be enthusiastic, self-motivated and hard-working, with a positive approach to delivering high quality care to the older person and be willing to work collaboratively with carers and other professionals. You will carry a defined caseload and manage this with the support of the team leader and the wider team.

There will be opportunities for you to develop your own clinical assessment skills, whilst risk assessment and management will remain at the forefront of everything you do. Being a learning organisation, we are happy to support you in advancing your own skill set using additional learning opportunities.

The post holder will hold a UK driving licence and have their own transport.

Main duties of the job
We work with a multitude of services across the Poole Locality to ensure the mental health needs of the older adult population is met.

This job is challenging but the team is well established and supportive. We are looking for someone who is reliable, committed and motivated with excellent communication and organisational skills and the ability to prioritise their caseload alongside balancing service demands.

The post holder will work as part of an established integrated multi-disciplinary team including Psychiatrists, Nurses, Occupational therapist, Psychologist, Social Services , Support Time Recovery Workers and an administration team .

The primary role responsibility is to act as dedicated Care Coordinator for a caseload of patients with whom you would provide person centred, holistic care to assist in aiding their wellbeing and recovery.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.

Person Specification
Knowledge, Skills and Training
  • Registered Mental Nurse
  • Valid registration with NMC
  • Evidence of continuous professional development
  • Valid RMN registration

Job Specific Experience
  • Able to assess, plan, implement and evaluate programmes of care.
  • Risk assessment and management
  • The ability to prioritise and meet deadlines; organise work of self and others; supervising, teaching and assessing; self-motivated with the ability to motivate others; effective resource management.
  • Skills in managing complex cases.
  • Works well in a multi-disciplinary team.
  • 1 years experience post qualification

Desirable
  • Uses initiative and can make decisions. Manages difficult situations effectively.
  • Awareness of Primary Care and Steps to Wellbeing.
  • Applies current research and theory into practice.

Information Technology
  • Competent computer skills, experience inputting information on RIO

Additional requirements
  • Demonstrable skills in written and spoken English, adequate to enable the post holder to carry out the role effectively
