This is an exciting opportunity for an ANP who is passionate about providing high-quality, patient-centred care and enjoys working as part of a wider multidisciplinary team.
The successful candidate will provide autonomous assessment, diagnosis and management of patients with a range of acute and long-term conditions. You will use your advanced clinical skills to support patients, improve access to care and work alongside our GPs, nurses and wider practice team.

Main duties of the job- Assessing, diagnosing and treating patients with a range of acute and long-term conditions.
- Carrying out clinical assessments and making appropriate treatment decisions.
- Prescribing and managing medicines within their scope of practice.
- Supporting patients with ongoing health needs and care plans.
- Providing health advice, preventative care and health promotion.
- Working closely with GPs, nurses and the wider healthcare team to provide joined-up patient care.
- Making referrals where appropriate and following clinical pathways.

Job descriptionJob responsibilitiesThe Advanced Nurse Practitioner (ANP) will work as an autonomous practitioner within Hawkinge & Elham Valley Practice, providing high-quality clinical care to our patient population.
The post holder will assess, diagnose and manage patients presenting with a wide range of acute and long-term conditions, using advanced clinical skills and professional judgement. The ANP will work collaboratively with GPs, nurses and the wider multidisciplinary team to improve patient access, outcomes and experience.

Key ResponsibilitiesClinical Care- Work autonomously to assess, diagnose and treat patients within your scope of practice.
- Manage a range of acute presentations and long-term conditions.
- Undertake clinical assessments, examinations and reviews.
- Develop and implement appropriate treatment plans.
- Prescribe medications safely in line with current legislation and professional guidance.
