Care Coordinator - Bruton Surgery, Nr Wincanton

BrutonSurgery, part of the Rural Primary Care Network (PCN) and Symphony Healthcare Services, is currently recruiting for a caring and compassionate Care Co-ordinator to join their Health and Wellbeing Team, with a focus on supporting our most vulnerablepatients within our community.

CareCo-ordinators help to navigate care across the health system, helping peoplemake the right connections and with the right teams at the right time. They can support people to become more activein their own health and care, and are skilled in assessing peoples changingneeds. Care Co-ordinators are effectivein bringing together multidisciplinary teams to support peoples complex healthand care needs.

This is a permanent, part post of 15 hours per week. Days and hours of work will be discussed at interview stage.

The rate of pay is £12.95 per hour, with the option to join the NHS or NEST pension scheme, plus other great employee benefits.

Main duties of the job

As a Care Co-Ordinator, you will focus specifically on the needs of our elderly and frail patients andtheir families, to provide support at a time which is most difficult andchallenging for any family.

Workingclosely with our Clinical Team, you will support patients by providingappropriate advice about services available, review and discuss with thepatient and their family their plans for the future, liaise with external teamssuch as Hospice and Complex Care as needed, and support our weekly carehuddles.

Understanding the particularchallenges for elderly and frail patients and their family, and services withinthe NHS would be an advantage.

The successful candidate will be kind, reflective and self-aware, and willenjoy working with a wide range of people.They will need have good communication and negotiation skills and beproactive with a flexible attitude, keen to work as part of a team andcommitted to developing themselves in this skilled role.

Job responsibilities

The post holder will operate as part of the PCN Personalised Care Group but will be based in one of the PCN practices.

Care coordinators play an important role within a PCN to proactively identify and work with people, including the frail / elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

They work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.

This is achieved by bringing together all the information about a persons identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person.

Care coordinators, review patients needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate.

• Experience of administrative duties Working in a multi-disciplinary setting where influence and negotiation is required Knowledge / familiarity with medical terminology
• Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field. Understanding of current issues facing the NHS Understanding of health and social care processes
