Clinical Nurse Specialist – Community Services Team

We are a patient-centred charity that delivers palliative and end of life care to those with advanced life-limiting illnesses, who live in Northwest Surrey. We care for patients and provide support to families and carers in the community as well as in our In-Patient Unit and Well Being Centre at the Hospice.

Clinical Colleague

Responsibilities :

• Provide a high level of clinical expertise, to support and facilitate individualised patient centred care to patients with life limiting illness
• Provide care and support for relatives and carers in collaboration with the MDT.
• Facilitate and support the implementation of evidence-based practice and contribute to the ongoing development of the service.
• Participate in a seven day per week operational service.
• Demonstrate leadership through clinical expertise and specialist knowledge, delivering high standards of personalised care.
• Identify needs through a holistic assessment and co-creating a care and support plan with people living with cancer and other life limiting illnesses.
• Independently manage own caseload of patients and associated responsibilities.
• Independently manage the role of duty nurse on rotation with colleagues
• Assist with the smooth running of the Community Team as a team lead in consultation with the Community Services Manager.

Minimum Requirements :

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
