Job description

Description
We are seeking a dedicated and experienced Patient Safety Lead to assist in delivering our commitment to safe, high-quality care. Working within the Clinical Governance team, you will support risk management processes, coordinate patient safety incident investigations, and contribute to the development of policies and audits.

This role involves gathering and analysing safety data, identifying trends, and helping to implement improvements that enhance patient outcomes. Strong organisational skills, attention to detail, and a collaborative approach are essential.

St John & St Elizabeth Hospital is a leading private hospital in London, offering a comprehensive range of specialist medical treatments, clinics and diagnostic services from a team of world-class consultants, supported by a dedicated team of clinical and non-clinical staff.

We are known for our exceptional standards of care, excellent nursing standards, and cutting-edge medical treatments.

We have 170 years of history and are looking for people to join us for the next chapter. We are very unique as the only truly charitable private hospital in London and as a charity, every private patient treated helps support our on-site community hospice.

What are the Responsibilities for the Patient Safety Lead role?

  • Manage the day-to-day incident management process on Datix, ensuring incidents are reviewed, graded and allocated promptly and that incident handlers are supported to complete reviews within agreed timescales.
  • Undertake a daily review of newly reported incidents, identifying those requiring immediate escalation to the Patient Safety, Governance & Risk Manager, the Head of Patient Safety, Quality & Risk or the Chief Nursing Officer.
  • Ensure patient safety incidents are recorded on the national Learn from Patient Safety Events (LFPSE) service and, as delegated, that other external notifications (e.g. CQC, PHIN) are submitted accurately and on time.
  • Monitor open incidents and associated actions, following up overdue items and escalating where progress is not being made.
  • Support staff following an incident, ensuring they are aware of the support available to them and promoting a just and restorative culture.
  • Manage the receipt, dissemination and monitoring of safety alerts received through the Central Alerting System (CAS), including National Patient Safety Alerts, ensuring actions are completed within required timescales and evidence of compliance is held.
  • Analyse incident data to identify trends, themes and emerging risks, using statistical process control (SPC) and other appropriate methods.
  • Develop and deliver patient safety training, including incident reporting, use of Datix, Duty of Candour and PSIRF.

Am I the right person for the Patient Safety Lead role?

  • Educated to degree level or equivalent professional training, qualification or experience
  • Patient Safety Syllabus training Levels 1 and 2, or willingness to complete on appointment
  • Experience of incident management in a UK healthcare setting
  • Experience of undertaking or supporting patient safety investigations or learning responses
  • Experience of using an incident and risk management system such as Datix
  • Experience of analysing data and producing reports for governance meetings or committees
  • Experience of supporting Duty of Candour or involving patients and families following incidents
  • Understanding of human factors and systems thinking in patient safety
