  • Accurately document all care provided and observations in the patient’s electronic records.
  • Work independently on a one-to-one basis within the patient’s home, ensuring safety, dignity and professionalism at all times.
  • Provide overnight support to a single patient, offering companionship and monitoring for any signs of deterioration.
  • Escalate any concerns or changes in the patient’s condition to the out-of-hours service promptly.
  • Deliver sensitive and respectful care following a patient’s death, including personal care and preparation of the body.
  • Notify the out-of-hours service in the event of a patient passing during the night and complete all required documentation.
