Hospital Team Manager - Adult Social Care

Location: West London Pay Rate: £45 per hour Working Arrangement: Hybrid – 3 days per week in the office Job Description We are currently recruiting for an experienced Hospital Team Manager to join a busy Adult Social Care team based within a hospital setting in West London. This is an excellent opportunity for an experienced social work professional with a strong background in hospital social work, managing teams and supporting the safe and timely discharge of patients from hospital. The successful candidate will have proven hospital experience, strong leadership skills and the ability to support social workers in delivering high-quality assessments, care planning and discharge arrangements for adults with a range of needs.
