Clinical Nurse Specialist - OBISNHS AfC: Band 6

Main duties of the job • Undertake clinical risk assessment and management plans and develop within these, comprehensive multi agency • To be one of the nursing specialist within OBIS • To provide specialist nursing advice for offenders with learning disabilities. This includes direct work with service users and families, advice to colleagues, other service providers and carers. • To identify and provide training in response to specific service user needs within a variety of settings where the care of the individual is provided. • To carry out joint assessments with other disciplines / agencies where appropriate • To take part in research of the health needs of people who have a Learning Disabilities to assist in developing high quality services to meet the needs of the service users and to enable learning and development to encourage high performing teams. • To maintain a personal and professional profile to fulfil NMC Revalidation requirements. • To provide a person-centred approach integrated within the care planning approach, which optimises integration into community life. • To participate in and encourage a multi-disciplinary approach at all times. • To develop and maintain a culture in which service users, supported by their advocates, where appropriate, are able to share in decisions, which affect their lives. • •
