Are you an experienced nursing leader with a passion for delivering high-quality community care and developing services that help patients receive treatment closer to home?
We are seeking a Community Matron to join our Community Referral Hub / Single Point of Access, providing senior clinical and professional leadership across Unplanned Care Services, with a particular focus on our community antibiotic services, including IVOPAT and COPAT.

- Provide specialist nursing advice and clinical oversight for patients with complex needs, supporting staff to recognise deterioration, manage risk and seek appropriate medical input.
- Lead and oversee community antibiotic services, including caseload coordination, treatment monitoring, weekly patient reviews and multidisciplinary meetings, ensuring decisions requiring infection expertise are escalated promptly to the specialist consultant.
- Lead clinical quality and governance, overseeing relevant incidents, complaints, risks and patient feedback, and translating findings into learning and measurable improvements.
- Lead service improvement and pathway development, using evidence, audit, patient experience and performance data to identify priorities and evaluate change.
- Support effective workforce and resource management, working with operational managers on staffing, recruitment, retention, staff wellbeing, absence management and budgets within the role's remit.
- Maintain professional standards and accountability, ensuring accurate records, compliance with Trust policies and NMC requirements.
