Transition Clinical Nurse Specialist RENAL
- Part-time
Job overview
Healthcare transition is the purposeful, planned process of supporting young people with ongoing health needs as they move through adolescence and towards adulthood. It aims to help young people progressively develop the knowledge, confidence, skills and independence needed to understand and manage their health and healthcare, while supporting parents/carers as their role changes. The role in Renal transition is essential in providing safe clinical support to our young people entering adult care.
Main duties of the job
Support young people and families to build confidence and independence while also coordinating safe, planned transfer to adult renal care.
More complex transition journeys: post-transplant, peritoneal dialysis (PD), home haemodialysis (HHDx), in-centre dialysis; plus referrals from other renal clinics where young people have multiple specialties, learning disability, or are known to the psychosocial team.
Post-renal transplant pathway identifies referral to the Healthcare Transition CNS at around 15–16 years.
Increasingly embedded into existing clinics, with follow-up phone calls where more in-depth discussion is needed. Support is bespoke rather than one universal pathway.
Young person (YP), parents/carers, renal specialty teams, renal admin, adult renal teams, other GOSH specialties, psychosocial support and GP/primary care for complex multi-team patients.
