This is an exciting opportunity to work within a dynamic multidisciplinary team, providing assessment, care coordination, therapeutic interventions, and recovery-focused support to service users in the community. As a Mental Health Practitioner, you will act as a Lead Professional for a caseload of individuals with complex mental health needs, ensuring the effective implementation of the Care Programme Approach (CPA). You will work collaboratively with service users, carers, and partner agencies to promote recovery, independence, choice, and personalised care. You will provide comprehensive assessments, develop and review care and risk management plans, facilitate multidisciplinary meetings, and deliver evidence-based interventions that support positive outcomes.
