The role\n
\n
\n As an Adult Locality Social Worker, you’ll support adults with a range of needs to live as independently and safely as possible in their own homes and communities. Your responsibilities will include:\n
\n \n

\n
• Completing Care Act assessments, reviews and support plans
\n
• Managing complex caseloads within the community
\n
• Undertaking safeguarding enquiries and risk assessments
\n
• Working closely with families, health professionals, providers and partner agencies
\n
• Supporting adults to access the right care, services and resources
\n
• Promoting independence, choice and positive outcomes
\n

\n
What you’ll need\n
\n \n

\n
• A recognised Social Work qualification
\n
• Active Social Work England registration
\n
• Experience in Adult Social Care, preferably within a locality or community team
\n
• Confidence managing a busy caseload and undertaking community visits
\n
• Strong assessment, safeguarding and care-planning skills
\n
