About the Role
We are seeking a highly motivated and experienced clinician to join our Early Supported Discharge (ESD) Service. In this role, you will provide specialist neurological assessment, treatment, support, and advice to individuals recovering from stroke, enabling them to achieve their rehabilitation goals within their home and community settings.

You will also provide professional and clinical leadership to registered practitioners and support workers, helping to develop a skilled and supportive workforce that delivers high-quality stroke rehabilitation services.

Main duties of the job
As a Specialist Community Physiotherapist, you will deliver a specialist physiotherapy service for stroke patients within the Early Supported Discharge pathway, providing high-quality assessment, treatment, and rehabilitation in community settings. You will undertake comprehensive clinical assessments and develop person-centred rehabilitation plans, support timely hospital discharge, and empower patients and their families in care planning and decision-making. Additionally, you'll monitor, evaluate, and adapt treatment programs, ensure the safe use of rehabilitation equipment, work collaboratively within a multidisciplinary team, and provide specialist stroke advice and support to colleagues across integrated community teams and partner services.

Key Responsibilities

• Deliver a specialist physiotherapy service for stroke patients within the Early Supported Discharge (ESD) pathway, providing high-quality assessment, treatment, and rehabilitation in community settings.
• Undertake comprehensive clinical assessments and develop person-centred rehabilitation plans that promote recovery, independence, and quality of life.
• Support timely hospital discharge and prevent unnecessary admissions by enabling safe and effective rehabilitation within patients' homes and local communities.
• Empower patients and their families to actively participate in care planning and decision-making, ensuring goals reflect individual needs and aspirations.
• Monitor, evaluate, and adapt treatment programmes to maximise rehabilitation outcomes, maintaining accurate and up-to-date clinical records.
• Assess, prescribe, and provide training in the safe use of rehabilitation equipment to patients, carers, and colleagues as appropriate.
• Work collaboratively within a multidisciplinary team, contributing to case reviews, MDT meetings, and integrated care planning to ensure seamless patient care.
• Provide specialist stroke advice, guidance, and support to colleagues across integrated community teams and partner services.
• Act as a clinical role model, providing supervision, mentorship, and support to junior staff, rehabilitation support workers, and students to promote high standards of practice.
• Contribute to service development, quality improvement initiatives, and the implementation of innovative practices that enhance stroke rehabilitation services across Wiltshire.

Essential

• Degree in Physiotherapy (BSc/BSc Hons or equivalent), HCPC registration, and relevant post-registration training.
• Experience working with adults with neurological conditions, including stroke rehabilitation, delivering patient-centred assessment, treatment, and case management.
• Experience working within multidisciplinary teams, collaborating across services, and building effective relationships with patients, carers, and partner organisations.
• Demonstrable leadership, supervision, teaching, or mentoring experience, with a commitment to supporting the development of colleagues and students.
• Strong clinical reasoning, communication, organisational, and IT skills, with the ability to manage a varied caseload, maintain clinical records, and communicate sensitive information effectively.
• Evidence of continuing professional development, evidence-based practice, and a commitment to maintaining high standards of care.

Desirable

• Experience working within community-based rehabilitation or Early Supported Discharge services.
• Involvement in service development, quality improvement, clinical audit, or implementing changes to practice.
• Experience working with a broad range of patient groups and external agencies to support integrated care pathways.
• Evidence of an innovative and flexible approach to service delivery and improving patient outcomes.
