Job description
The following content displays a map of the jobs location - Dartford As a Band 5 Community Staff Nurse, you will join our valued Swanley and Rural PCN Community Nursing Team in North Kent. If you are newly qualified, you will benefit from our award‑winning Preceptorship Programme, combining face‑to‑face learning, e‑learning and practical support to help you develop confidence in your role. This is a full‑time position working 37.5 hours per week. The service operates seven days per week between 8:00am and 8:00pm.A full UK driving licence and access to a vehicle for work purposes is essential. Working closely with Senior Sisters, multidisciplinary colleagues, patients and carers, you will help deliver safe, effective and evidence‑based care tailored to each individuals needs. You will manage your workload independently while contributing to the wider team, ensuring patient care is prioritised appropriately and any changes in a patients condition are identified and escalated promptly. You will also play an active role in health promotion, service improvements, clinical audits and the ongoing development of community nursing services. Key responsibilities include: * Delivering high‑quality, patient‑centred nursing care in community settings * Participating in patient assessment, care planning and evaluation * Supporting patients with acute, chronic and complex healthcare needs * Working collaboratively with GPs, therapists, social care colleagues and wider multidisciplinary teams * Maintaining accurate and timely patient records using electronic systems * Promoting independence, health and wellbeing through education and health promotion * Participating in team meetings, clinical supervision and professional development activities * Contributing to service improvement initiatives, audits and evidence‑based practice * Supporting student nurses and colleagues as your experience develops Person Specification * Registered Nurse with current NMC registration * Diploma level qualification or equivalent * Evidence of continuing professional development * Strong communication and interpersonal skills * Ability to work both independently and as part of a multidisciplinary team * Excellent organisation and prioritisation skills * Commitment to delivering high‑quality, evidence‑based patient care * Confidence using IT systems and electronic patient records * Full UK driving licence and access to a vehicle for work purposes * Previous community nursing experience * Mentorship or teaching qualification, or willingness to undertake training * Understanding of current developments within community healthcare and integrated care services About the Company We change lives by transforming health and care.
