Job description
The following content displays a map of the jobs location - Gravesend As a Band 4 Associate Practitioner, you will join our valued Garden City Team based at Gravesham Community Hospital. This is an exciting opportunity for an experienced Healthcare Assistant, Senior Healthcare Assistant or Associate Practitioner looking to progress their career within community nursing. Working alongside Community Nurses and the wider multidisciplinary team, you will play a key role in delivering high-quality clinical care to patients in their own homes and community settings. You will support patients with a range of healthcare needs, undertaking delegated clinical interventions and monitoring patient wellbeing while helping to deliver personalised, patient-centred care. This is a varied and rewarding role that offers autonomy, responsibility and the opportunity to further develop your clinical skills within a supportive team environment. You will be expected to work independently within your scope of practice while recognising when concerns need escalating to a registered clinician.You will play an important role in monitoring patient progress, maintaining accurate records and supporting patients to remain safe and independent within their own homes.Key responsibilities include: * Delivering delegated clinical care to patients within community settings * Undertaking clinical observations and monitoring patient wellbeing * Supporting medication administration and medication management in line with competency and organisational policies * Delivering wound care and supporting ongoing treatment plans * Escalating changes in patient condition to registered clinicians appropriately * Maintaining accurate and timely clinical documentation * Supporting patients with long-term conditions and complex healthcare needs * Working collaboratively with Community Nurses, GPs and the wider multidisciplinary team * Providing patient education and promoting health and wellbeing * Contributing to service improvement initiatives and maintaining high standards of care Due to the community nature of this role, a full UK driving licence and access to a vehicle is essential. Person Specification * NVQ Level 3 in Health and Social Care or equivalent healthcare qualification * Completion of an Associate Practitioner qualification, or willingness to undertake Associate Practitioner training (minimum NVQ Level 3 qualification required) * Experience working in a healthcare setting, ideally within community nursing, district nursing or integrated care services * Experience supporting clinical interventions such as wound care, medication administration or long-term condition management * Ability to work independently and prioritise workload effectively * Strong clinical observation and monitoring skills * Excellent communication and interpersonal skills * Ability to build positive relationships with patients, carers and colleagues * Knowledge of confidentiality, safeguarding and information governance * Confidence using IT systems and electronic patient records * Full UK driving licence and access to a vehicle for work purposes * Previous experience in a Band 3 Healthcare Assistant, Senior Healthcare Assistant or Assistant Practitioner role * Experience within community healthcare services * Venepuncture competency * Experience supporting patients with complex healthcare needs * Foundation Degree in a relevant subject About the Company We change lives by transforming health and care.
