What you will do * Assess, diagnose, and tailor treatment plans for patients with a variety of conditions, including acute medical, respiratory, neurological, orthopaedic, and elderly care. * Deliver evidence-based interventions and programmes aimed at effective patient rehabilitation. * Conduct mobility, balance, respiratory, and functional independence assessments to guide care. * Collaborate extensively with consultants, nurses, occupational therapists, and other members of the multidisciplinary team in patient‑centred care. * Facilitate effective discharge planning and make necessary onward referrals to maintain continuity of care. * Uphold high standards in clinical record-keeping in line with HCPC and Trust guidelines. * Engage in weekend and on-call services to ensure comprehensive patient care.
