What you will do: * Conduct comprehensive assessments of patients recovering from strokes, identifying their individual needs and challenges. * Develop and implement personalised treatment plans aimed at enhancing patients’ independence and quality of life. * Collaborate with a multidisciplinary team to coordinate effective care and ensure seamless service delivery. * Engage in discharge planning, liaising with patients, families, and community services to ensure smooth transitions. * Provide expert guidance and support to junior staff, contributing to the professional development of the team. Requirements: * Registration with the Health and Care Professions Council (HCPC). * A minimum of 2 years’ experience working as an Occupational Therapist, preferably in an inpatient rehabilitation setting. * Experience in stroke management and discharge planning. * A car driver is preferable for visits and community-based interventions.
