Single Homeless Project has an opportunity for an Outreach Housing Pathway Coordinator to join and work in our team based in Camden. You will join us on a full time, permanent basis and in return, you will receive a competitive salary starting at £34,127.11 and rising incrementally to £36,947.49 per annum.

About the Outreach Housing Pathway Coordinator role:

\n

Rough sleeping pathways only work when the system around people moves with urgency, consistency and accountability. This role is about making sure people sleeping rough, or at risk of returning to the streets, are not left waiting, drifting between services, or losing accommodation because barriers have not been gripped quickly enough.

\n

As Outreach Housing Pathway Coordinator, you will work within Camden’s integrated Rough Sleeping and Outreach Hub to help people access and sustain appropriate accommodation. You will hold oversight of where people are in their housing journey, identify what is blocking progress, and work with outreach teams, Navigators, hostels, pathway providers, community safety partners and statutory services to keep plans moving.

You will focus on the points where pathways can break down: delays, exclusions, unsuitable offers, unresolved risks, street activity linked to accommodation, or support needs that have not yet been fully understood or responded to. Through assertive engagement, practical case coordination and strong partnership working, you will help people stabilise their accommodation and reduce the risk of returning to rough sleeping.

Working within Camden’s No Wrong Door model, you will bring grip, judgement and persistence to complex situations, ensuring people experiencing multiple disadvantage receive a response that is joined-up, trauma-informed and focused on lasting routes away from the street. SHP will support you with the training, reflective practice and development opportunities needed to continue building your expertise in this work.

About you:

  • You understand that people experiencing rough sleeping are often navigating trauma, exclusion, unmet needs and systems that have not worked for them. You bring patience, persistence and a clear belief that people should not be written off when engagement is difficult.
  • \n
  • You know how homelessness pathways, outreach services and supported accommodation can work together, and you can spot where delays, exclusions or gaps in communication are putting someone’s route away from the street at risk.
  • \n
  • You are confident working with complexity. You can hold risk, need and accountability together, making balanced decisions while keeping people’s safety, dignity and long-term accommodation sustainment at the centre.
  • \n
  • You build relationships that help things move. Whether you are speaking with someone on the street, a hostel, a Navigator, a statutory service or a community safety partner, you communicate clearly, follow through on actions and keep people focused on solutions.
  • \n
  • You bring practical judgement, emotional resilience and a trauma-informed approach to your work, with the ability to stay calm and purposeful when situations are challenging, stuck or urgent.

Our attractive benefits package includes:

  • A salary increase after successfully completing six month's probationary period

  • A 37.5 hour working week including flexible working hours (core hours are 10am – 4pm) in non-accommodation services
