Job description
Are you passionate about making a real difference to the lives of people living with a neuromuscular condition? This is an exciting opportunity to join the specialist Neuromuscular Service at The Walton Centre, as an Adult Neuromuscular Care Advisor. Working alongside a highly skilled multidisciplinary team, you will help people navigate complex health and social care systems, coordinate personalised care, influence service development and promote best practice in neuromuscular care. This is a unique opportunity to combine clinical expertise, patient advocacy and service improvement within a nationally recognised specialist service. Main duties of the job * Provide specialist care coordination for adults with neuromuscular diseases across Cheshire, Merseyside, Lancashire and the Isle of Man, ensuring care delivery aligns with national neuromuscular standards and supports transition from children’s to adult services where appropriate. * Offer holistic support to patients and families across the lifespan, delivering emotional and practical guidance, signposting to health, social care and voluntary services, and contributing to personalised care and intervention planning for progressive and life ‑ limiting conditions. * Work collaboratively with multidisciplinary teams and partner organisations, including NHS services, social care, and the voluntary and independent sector, to promote consistent, high ‑ quality care, shared expertise, and improved outcomes for people affected by neuromuscular conditions. Originally formed in 1992, the Trust received Foundation Trust status in 2009. With around 1,450 staff, The Walton Centre treats more than 127,000 outpatients and 18,000 inpatients each year. We have leading specialists and incredibly dedicated staff delivering excellent clinical outcomes for brain, spinal and neurological care nationally and internationally. Teams across our site in Fazakerley, Liverpool, offer a world‑class service in diagnosing and treating injuries and illnesses affecting the brain, spine and peripheral nerves and muscles, and in supporting people suffering from a wide range of long‑term neurological conditions. Detailed job description and main responsibilities * Act as specialist neuromuscular care coordinator, planning, delivering and reviewing care in line with best practice. * Serve as a primary contact for patients, families and carers, providing information, advice and support throughout the care pathway. * Coordinate care across multiple settings (secondary, community, primary care, social care and voluntary sector). * Support transition from paediatric to adult services. * Develop, implement and review personalised care plans, including anticipatory and symptom management. * Facilitate MDT working and shared decision‑making. * Refer and signpost to appropriate services based on need. * Promote equitable access, service consistency and pathway improvement. Person specification Qualifications/Professional Registration * Educated to degree level or equivalent in nursing, social work, occupational therapy, or physiotherapy * Masters level qualification or evidence of equivalent experience * Evidence of continuing professional development (CPD) relevant to clinical practice * Commitment to maintaining professional registration, revalidation and scope of practice * Additional training in neurology, neuromuscular disease, long‑term conditions, advanced clinical practice or care coordination Experience * Substantial post‑registration experience working with adults with long‑term, complex or neurological conditions * Experience of autonomous practice and managing complex caseloads * Experience of working within a multidisciplinary and/or multi‑agency setting Knowledge * In‑depth understanding of the complex needs of people with long‑term and progressive neurological conditions * Knowledge of national standards, frameworks and best practice guidance relevant to neuromuscular and long‑term conditions * Understanding of clinical governance, safeguarding, risk management and information governance * Knowledge of local and national health, social care and voluntary sector services * Knowledge of specific neuromuscular conditions and their clinical, psychological and social impact * Understanding of anticipatory care, palliative approaches and end‑of‑life planning * Knowledge of the benefits system, with the ability to support patients in accessing and applying for appropriate entitlements. Skills & Abilities * Highly developed clinical assessment and clinical decision‑making skills * Ability to manage complex, unpredictable and emotionally challenging situations * Excellent communication and interpersonal skills, including influencing and negotiating across services * Excellent listening skills, with the ability to communicate effectively with individuals who have learning disabilities, impaired communication, or complex emotional and physical needs (including speech, hearing and sight impairments). * Ability to coordinate complex care pathways and prioritise workload effectively * Ability to work independently and autonomously, including across a wide geographical area * Ability to critically appraise and apply evidence‑based practice * Teaching, training or facilitation skills * Experience presenting information to groups or professional audiences * Understanding of the education system, including SEND provision andnEducation, Health and Care Plans (EHCPs). Leadership & Management * Experience supporting, advising or supervising staff, students or peers * Ability to contribute to service development, pathway design and quality improvement * Acts as a professional role model, demonstrating Trust values and behaviours * Ability to provide clinical leadership within a specialist service * Experience leading audit, service evaluation or project work * Experience representing a service at local, regional or national meetings Clinical Governance & Professional Practice * Commitment to clinical governance, audit, service evaluation and continuous quality improvement * Adherence to professional codes of conduct and ethical practice * Understanding of health and safety responsibilities * Compliance with information governance and Caldicott principles Personal Attributes/Other Requirements * Compassionate, resilient and adaptable * Able to manage the emotional demands of working with complex and life‑limiting conditions * Flexible approach to service delivery * Willingness and ability to travel across the Walton Centre service footprint as required Employer certification / accreditation badges You must have appropriate UK professional registration.
