Job summary
This is an exciting opportunity to join a well-established specialist community service within the Neurodiversity services in NE Lincolnshire to provide support for service users within the Forensic Outreach Liaison Service (FOLS).

The local NE Lincolnshire FOLS Team is part of a wider FOLS Service working beside HUMBER, which means you will also be working alongside Specialist Forensic Services in Hull to support our Transforming Care partners to repatriate offenders with learning disabilities (LD) and/or Autism and complex needs back to their local communities.

The aim of the FOLS is to reduce or safely manage behaviour amongst people with LD and/or autism which may put members of the public at risk and would thereby otherwise lead to contact with the criminal justice system or admission to a secure hospital.

The initial focus of the service will be to support an identified patient cohort transition effectively from secure care into community provision; safely managing risk and avoiding contact with the criminal justice system or re-admission to secure hospital. The service will support a more effective approach to the identification and management of individuals at risk of offending or being admitted into secure services.

Range 7 £45,987 - £52,434 (pay award pending)

We are looking for an experienced and enthusiastic professional to join our Forensic Outreach Liaison Service (FOLS)

You will:
  • Manage a specialist caseload, provide expert advice on risk assessment and management
  • Contribute to safe, effective, and person-centred care
  • Support and develop colleagues through training, consultation, and professional leadership, helping to enhance skills and confidence
  • Contribute to service development, quality improvement and the implementation of best practice
  • Help ensure positive outcomes for service users while promoting recovery, safety, and community inclusion

A relevant professional registration with good working knowledge of the Mental Health Act and health & social care pathways is essential as well as experience working with complex cases in the field of LD and/or Autism.

Good communication and organisation skills are also essential as well as the ability to work autonomously and as part of a Multi-Disciplinary Team working in partnership with other services to meet the Transforming Care targets and to provide a successful community based forensic support service that improves the health and wellbeing of local people.

Following the shortlisting process, successful applicants will be invited to deliver a formal presentation, which will be followed by a panel interview at the Val Waterhouse Centre, Grimsby.

Essential
  • Effective team building, developing positive interpersonal relationships,
  • Effective negotiation, conflict resolution and problem solving skills
  • Experience of managing change within the workplace
  • Experience of working with challenging behaviour, in physically and emotionally demanding environments.
  • Evidence of advanced practice skills and able to demonstrate the impact of this on practice change/development

Qualifications
Essential
  • Hold a relevant Professional Qualification RNLD/RMN, DipOT/BsOT, DipSW with current registration
  • Masters Level/Post Graduate advanced clinical/professional qualification - or be able to fully demonstrate equivalent knowledge and experience
  • Leadership qualification - or be able to demonstrate advanced leadership and management skills

Desirable
  • Relevant forensic experience in a range of settings
  • Trained to complete Specialist Risk Assessments (HCR-20, RSVP, SAPROF)

Skills and Knowledge
Essential
  • Evidence of sound post-registration professional practice
  • Full understanding of relevant policy, legislation, drivers and their application to clinical and service area
  • Full understanding/application of relevant clinical practice/standards/audit within identified clinical area
  • Detailed understanding of clinical governance
  • Thorough understanding of good practice in risk assessment, formulation and management plans
  • Ability to understand and translate national directives and agendas into appropriate local strategies and plans
  • Advanced critical thinking and diagnostic reasoning skills
  • Proven ability to assess and interpret a range of complex facts and situations to inform the planning of appropriate interventions
  • Advanced knowledge of Mental Health legislation relevant to the clinical area, in particular parts 2 & 3 of the MHA.
  • Evidence of project work that has impacted on practice
  • Ability to effectively communicate complex and detailed information to a wide range of recipients including service users.
  • Ability to lead and motivate staff
  • Up to date awareness of evidence based good practice in the field of LD/ Risk management
  • A comprehensive understanding of clinical governance in the context of service improvement.
  • A thorough working knowledge of issues of confidentiality in regard to the provision of care and public protection

  • Experience working in forensic services
  • Understanding of the criminal justice system
