Job summary
The post holder will provide strategic, professional, and clinical leadership for Specialist Palliative Care and End of Life Care services across the North and East locality. They will lead and support service development and quality improvement initiatives, ensuring the delivery of high-quality, patient-centred care.

The role includes the operational management of a team of Clinical Nurse Specialists, providing effective leadership, supervision, and support while leading by example in clinical practice.

The post holder will act as an expert resource and advisor for Specialist Palliative Care and End of Life Care, offering guidance and support to health and social care professionals across the locality.

Working collaboratively with senior managers and the End of Life Facilitator, the post holder will ensure that robust governance, monitoring, and evaluation systems are in place to support the implementation of national and local guidance, policies, standards, and strategies relating to Specialist Palliative Care and End of Life Care. They will promote continuous improvement, drive service innovation, and ensure that outcomes for patients, families, and carers are regularly reviewed and enhanced.

The post will be based at Liskeard community hospital primarily, but applicants will be expected to travel and spend time at other locations around the county for meetings and other events.

Main duties of the job
  • Manage a geographically defined caseload of patients with complex palliative and end of life care needs, using recognised referral criteria and agreed levels of intervention.
  • Provide comprehensive specialist assessments, symptom management, psychological support, and advice tailored to individual patient needs.
  • Work collaboratively as part of a multidisciplinary team and in partnership with other Specialist Palliative Care providers to ensure coordinated, high-quality care.
  • Participate in a seven-day service rota to provide responsive specialist palliative care support.
  • Demonstrate advanced clinical decision-making and autonomous practice in the management of complex patient needs.
  • Support and improve standards of care through clinical supervision, audit, research, teaching, and service evaluation.
  • Provide specialist advice, support, and education to health and social care professionals to promote best practice in palliative and end of life care.

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

Education / Qualifications and Relevant Experience
Essential
  • Current professional registration -Registered Nurse
  • Professional qualification - Diploma / Degree
  • Evidence of further formal study, eg. Post Graduate MSc qualification/module, relevant palliative care course
  • Evidence of broad range of senior experience in palliative care
  • High level of skill and experience including supervising people including qualified and support staff
  • Experience leading a team
  • Proven experience in managing a change process

Knowledge and Skills/Skills and Aptitude
Essential
  • Specialist knowledge and application of specialist palliative assessments and interventions
  • Highly developed skills in clinical assessment, intervention and evaluation
  • Highly developed communication skills and ability to use most effective communication style for each context and culture
  • Basic management skills
