Job Introduction

Are you an experienced clinical pharmacist looking to play a key role in transforming care for older and frail patients closer to home?

We have an exciting opportunity to join our innovative Hospital at Home (H@H) service, supporting the delivery of high-quality acute care within patient's own homes across the BSW system. As a Clinical Pharmacist, you will be at the heart of a multidisciplinary team, helping to optimise medicines, improve patient outcomes and support personalised care for some of the most complex patients in our communities.

Working closely with clinicians, Advanced Clinical Practitioners and wider healthcare professionals, you will provide expert pharmaceutical advice, contribute to Hospital at Home MDT discussions, and support clinical decision-making that enables safe, effective and patient centred care. You will lead on medicines optimisation for older people living with frailty, ensuring prescribing is evidence-based, cost-effective and tailored to individual needs.

This role offers the opportunity to influence service development, contribute to clinical governance and safety initiatives, develop guidance and protocols, and support innovative models of care that reduce hospital admissions and improve patient experience. You will also play an important role in ensuring compliance with medicines legislation, regulatory standards and best practice guidance, helping to maintain the highest standards of pharmaceutical care across the service.

Main Responsibilities

As the Clinical Pharmacist for Hospital at Home, you will lead, deliver and continuously develop a safe, effective and high-quality clinical pharmacy and medicines optimisation service for patients receiving acute care at home.

Working as a key member of the multidisciplinary team, you will provide expert pharmaceutical advice, attend Hospital at Home MDT meetings, and support personalised care planning for older people living with frailty and complex health needs. You will champion safe, evidence-based and cost-effective prescribing, ensuring medicines are optimised to improve patient outcomes and experience.

The role also includes supporting clinical governance, developing policies and guidance, and working with pharmacy leadership colleagues to ensure compliance with medicines legislation, regulatory requirements and best practice standards, maintaining the highest quality of pharmaceutical care across the service.


The Ideal Candidate

Essential

  • Master’s degree in Pharmacy or equivalent (e.g. BSc/BPharm/MPharm), registered with the GPhC as a pharmacist
  • Clinical Pharmacy Diploma (2 years) or equivalent experience, with evidence of continuing professional development
  • At least 2 years’ experience working in a clinical pharmacy role, including experience working within a multidisciplinary team
  • Good knowledge of medicines legislation and national standards, and of medicines management issues relating to transfer between primary and secondary care
  • Knowledge of medication adherence/concordance and options to support safe management of medication by patients in the community
  • Understanding of clinical governance and risk management, and of prescribing and pharmaceutical systems/processes across care settings
  • Excellent clinical knowledge
  • Effective written and verbal communication skills in English; able to plan own work, work independently without direct supervision when required, and lead and work as part of a team
  • Empathic, diplomatic and able to respond professionally to emotional or confrontational situations, with flexibility in approach to meet service needs
  • Good manual dexterity and sufficient IT literacy to use computers in all aspects of the job, including a working knowledge of Microsoft Word, PowerPoint, Excel and Teams
  • Clinical audit skills; able to critically appraise and evaluate evidence, and to work alone
  • Able to identify and prioritise complex, time-sensitive workload and projects, and to deal with complaints and conflict professionally

Desirable

  • MSc in Clinical Pharmacy
  • Member of the Royal Pharmaceutical Society (RPS)
  • Independent Prescriber
  • Experience of community health and social care
  • Knowledge and experience of patient safety, specifically relating to medicines safety
  • Experience of service development and of managing change and/or service improvement
  • Familiarity with prescribing analysis ePACT2 data and GP clinical systems
  • A good understanding of drug budgets and how to interpret drug expenditure reports
  • Experience of training and mentoring pharmacists, pharmacy technicians and/or other healthcare professionals
  • Experience of inter-agency (partnership) working, and experience in Mental Health
  • Ability to write up and present projects and audit work; teaching and mentoring skills
  • Experience with Electronic Patient Records (e.g. SystmOne), Electronic Prescribing Systems and electronic safety systems (e.g. DATIX)
