Job summary
Middle Chare Medical Group provides high-quality, patient-centred care across two well-established surgeries. Our main site, Middle Chare Surgery, is located in a thriving market town with excellent transport links, including the London to Edinburgh East Coast Main Line and the A1(M), making it an ideal location for commuting. Our branch site, Woodlands Surgery, is situated in the welcoming village of Rickleton, Washington.

Together, we care for approximately 9,500 patients and pride ourselves on delivering high standards of clinical care in a supportive and collaborative environment. We are a friendly, forward-thinking practice with a Good CQC rating and are actively involved in research to help shape the future of primary care.

Our experienced multidisciplinary team includes five GPs, three Advanced Nurse Practitioners, and a highly skilled nursing team who provide comprehensive chronic disease management. We are also supported by a dedicated pharmacy team who manage medication queries, process discharge medications, and undertake structured medication reviews, allowing our clinicians to focus on delivering excellent patient care.

We use EMIS Web clinical systems and have developed strong working relationships fostering a collaborative approach to delivering the best possible outcomes for our patients.

Main duties of the job
As a General Practitioner, you will deliver high-quality, patient-centred care through a mix of face-to-face and telephone consultations, home visits, and clinical decision-making. You will work closely with our experienced multidisciplinary team to provide safe, effective care, manage a varied clinical workload, and contribute to the ongoing development of the practice. The role offers a diverse range of responsibilities in a supportive, collaborative environment and requires flexibility to meet the changing needs of a busy primary care setting.

Job responsibilities
The post holder will undertake the full range of duties expected of a General Practitioner working within primary care. In accordance with the agreed practice timetable, responsibilities will include:
  • Providing face-to-face, telephone and, where appropriate, digital consultations.
  • Assessing, diagnosing and managing patients presenting with acute and chronic medical conditions.
  • Triaging patients and making autonomous clinical decisions in relation to presenting problems, whether self-referred or referred by another healthcare professional.
  • Undertaking home visits where clinically appropriate.
  • Participating in the duty doctor rota, managing urgent clinical queries and same-day demand.
  • Reviewing, authorising and signing repeat prescriptions and responding to medication-related queries.
  • Reviewing and actioning pathology and investigation results in a timely manner.
  • Preparing accurate and timely referrals to NHS, private and voluntary sector services as appropriate.
  • Completing medical reports, insurance reports, medical examinations and other clinical documentation within agreed timescales.
  • Maintaining accurate, clear and contemporaneous electronic clinical records using EMIS Web in accordance with professional and legal standards.
  • Prescribing safely and cost-effectively in accordance with local and national prescribing guidance.
  • Supporting clinical audit, quality improvement initiatives and the collection of data where required.
  • Working collaboratively with the multidisciplinary team to deliver safe, effective and patient-centred care.
  • Undertaking any additional duties appropriate to the role, knowledge, skills and experience of the post holder, as agreed with the Partners, to meet the evolving needs of the practice.

Professional Responsibilities
The post holder will:
  • Comply with all practice policies, procedures, national guidance and statutory requirements.
  • Maintain a commitment to lifelong learning, continuing professional development, annual appraisal and GMC revalidation.
  • Contribute to clinical governance, audit, service evaluation and the continuous improvement of patient care.
  • Promote evidence-based practice and support the delivery of safe, high-quality healthcare.
  • Contribute to the ongoing development and effective use of electronic patient records and clinical systems.
  • Attend and participate in mandatory training, practice meetings, educational events and multidisciplinary meetings as required.
  • Complete medical reports and other professional documentation in a timely manner.
  • Maintain professional standards of behaviour, confidentiality and communication at all times.
  • Present a professional appearance and adhere to the practice dress code.

The post holder will be expected to work flexibly to meet the needs of the practice and may be required to undertake other duties commensurate with the grade and responsibilities of the role.

Qualifications
Essential
  • A vocationally trained and accredited GP MBBS or equivalent medical degree Full registration with GMC Certification of Completion of Training Inclusion on Performers List

Experience
Essential
  • A good knowledge of and interest in general practice and primary care Strong levels of IT proficiency and familiarity with primary care clinical systems (EMIS WEB). Experience in using the Microsoft suite of programmes Proven ability to handle a busy and varied primary care case load A good understanding of primary care contracting requirements Overview of working to achieve standards within the Quality and Outcomes Framework (QOF) Strong attention to detail Excellent verbal and written communication skills to interact with patients and other healthcare professionals Progressive and forward-thinking attitude

  • Understand the health and social needs of a local practice population Understanding of the current issues and challenges facing Primary Care Experience of supporting service change Experience of service improvement and quality initiatives
