Main duties of the job
As a Complex Care Nurse, you will coordinate and deliver comprehensive care plans using a holistic approach, including comprehensive geriatric assessments and medication reviews. You will work closely with multidisciplinary teams across primary, secondary, and specialist care services to enable patients' self-management and ensure seamless care.
