Share information relating to pharmaceutical products, medicines management, medicines optimisation, medicines legislation and general health education so that patients and colleagues are appropriately informed. You would be expected to be responsible for an area of chronic disease management.

Support the general practice staff with prescription and medication queries and help to enable partnership working with community pharmacies and secondary care.

The post holder will help to recognise and improve processes, supporting the team to continue to deliver excellent healthcare. Analysing data to produce reports and recommendations to support population health management, helping the team to complete projects through to a successful and timely conclusion.

About us
You will need to be flexible and used to working independently, whilst forming part of a wider team. Sherborne has a team of three Prescribing Pharmacists, and three Pharmacy Technicians who work the majority of their week at one of our three practices, serving a total patient list of approx. 25,000.
