EOI - Band 6 Cardiac Rehabilitation Specialist Nurse (Heart Failure) 12-Month Secondment

An exciting opportunity has arisen for a motivated Registered Nurse to join the Cardiac Rehabilitation Team as a Band 6 Cardiac Rehabilitation Specialist Nurse (Heart Failure). This 12-month secondment offers an excellent development opportunity for an individual with heart failure experience and a passion for supporting patients to improve their health and quality of life.

Secondment will be considered for internal candidates - If you are successful after interview, you must obtain approval from your current line manager to accept the offer.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Main duties of the job
Key Responsibilities
  • Assess patients with heart failure and monitor symptoms.
  • Support delivery of cardiac rehabilitation programmes.
  • Provide education, lifestyle advice, and self-management support to patients and carers.
  • Participate in nurse-led reviews and follow-up clinics.
  • Work collaboratively across acute, community, and primary care settings.

Essential Criteria
  • Registered Nurse with current NMC registration.
  • Significant post-registration experience in heart failure care.
  • Excellent assessment, communication, and organisational skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to evidence-based practice and professional development.

Job responsibilities
  • To work autonomously taking responsibility for care, quality and clinical activity for the inpatient caseload of patients.
  • To process referrals appropriately, ensuring that patients are seen within designated waiting times and that tasks are delegated to administration staff effectively and efficiently and communicated to wider multidisciplinary team.
  • To triage and prioritise patients, demonstrating the use of a variety of techniques to elicit the history of an illness and determine an appropriate care plan.
  • To take responsibility for direct patient care: assessing, planning, providing and evaluating nursing care needs including monitoring, diagnostics and investigations.
  • To facilitate appropriate clinical interventions for patients, to interpret and act on the results, working within local and national guidelines and where necessary refer to the appropriate team or general practitioner.
  • To undertake interventions consistent with evidence-based practice, transferring and applying knowledge and skills with wider multidisciplinary team members to meet patient needs.
  • To evaluate the effectiveness of interventions in achieving prior arranged targets and making any required changes.
  • To collaborate with patients and their carers to enable them to make informed decisions regarding their health and healthcare.
  • To support patients, their carers and relatives by providing up to date information on secondary prevention (which meet British Association of Cardiovascular Rehabilitation and Prevention Standards).

Person Specification
Qualifications - Essential
  • Current professional registration NMC.
  • A relevant post registration qualification/training and experience - eg: ENB 124, 249 or 254.
  • Evidence of recent relevant personal professional development.
  • The ability to work without direct supervision, yet as part of a team.
  • Study at level 6 (degree/diploma).

Qualifications - Desirable
  • ILS certification.
  • Degree level Heart Failure course eg: Caledonian and/or post graduate qualification in rehabilitation.

Experience - Essential
  • Recent experience within an acute cardiac setting.
  • Patient teaching/training - groups/individuals.
  • Multidisciplinary working.
  • Clinical audit and evaluation.

Knowledge - Essential
  • Cardiac Knowledge.
  • Evidence of managing a caseload.

Knowledge - Desirable
  • Education/teaching skills, providing and creating learning opportunities for students and the wider health team.

Skills - Essential
  • Excellent communication and interpersonal skills.
  • IT skills include Word, Excel spreadsheets, email, internet access.
  • Ability to work autonomously and caseload management ability.
  • Excellent clinical assessment skills.
  • Proven written and oral communication skills including the ability to deal with challenging and sensitive situations.
  • Self-motivated and self-disciplined.
