Job summary
The post carries overall responsibility for ensuring effective and efficient management of the cardiac surgery waiting lists including appropriate scheduling to ensure that the theatre and critical care resource is used to its maximum and supporting the delivery of local activity delivery plans. The clinical case manager will be responsible for patients referred to the cardiac surgery according to agreed criteria and is responsible for ensuring patients have the appioriate assess,emts and tests prior to surgical intervention.

The postholder will provide specialist expertise, advice, education and support to patients, their carer's and other health care professionals, and advise and support the wider cardiac surgery team regarding the overall clinical management of the service.

In addition, the post holder will work across professional, departmental and Trust boundaries as required to develop and support patient-centred seamless care, with responsibility for leading new care initiatives within their speciality.

The clinical case manager will work closely with consultant cardiac surgeons, registrars, and the cardiac surgical enhanced practitioners' team to ensure that patients are fully optimised, appropriately investigated, and efficiently progressed through the multidisciplinary team (MDT) and elective or inpatient surgical pathways.

Main duties of the job
To manage the cardiac surgery patient caseload to agreed referral / discharge criteria and provide specialist nurse-led intervention, as part of co-ordinated cardiac pathway of care.

To clinically manage the cardiac surgery waiting lists, liaising with the performance manager to ensure that activity is in line with local delivery plans.
  • Act as central clinic coordinator for cardiac surgical referrals from external hospitals, using referapatient.
  • Ensure timely progression of accepted patients into the tertiary cardiac surgical pathway, and reduce length of stay and minimise by the cancellation of elective and non elecive patients
  • Coordinate clinical information transfer between referring hospitals and MDT teams, inc. management plans for patients awaiting surgery
  • Work with Band7 EPs and cardiac consultants to work identify patients who are suitable for a cardiac virtual ward.
  • Ensure completion of all prerequisite investigations prior to transfer, including Echo, Coronary angiogram, CT, routine blood test, Group& save and dental assessment,
  • Highlight delays / long waiters, both accepted and not accepted by cardiac surgery.
  • Attend and coordinate patients for cardiac surgery MDT & attend Aortic MDT.
  • Suppport delivery of timely complex anaesthetic assessment
  • Dr Dr video link to referral centres if patient suitability/ complexities are flagged.
  • Ensure MDT decisions are actioned promptly and communicated clearly via referapatient.

Job responsibilities
  • Support prioritisation of cases based on clinical need.

To establish nurse-led interventions to support services and manage patients needs and expectations such as nurse led discharge.

To support B7 EPs to establish links with other organisations providing support and information and utilise these additional resources where necessary.

To adhere to infection control guidelines and procedures.

To work clinically as part of the nursing team, to share experience, knowledge and expertise and role modelling to more junior staff. In line with the divisional approach.

To provide support and advice in clinical areas, wards and outpatients, to optimise quality improvement opportunities. This may involve working alongside nurses in the clinical areas.
  • Work collaboratively with pre-operative assessment services (C503 cohort) and waiting list teams to improve the patient case management and pathway through the service.
  • Prevent premature listing of patients who are not fully work-up complete.
  • Reduce avoidable cancellations, delays, and extended inpatient waiting times.
  • Clinically assess patients on inpatient ward areas as part of their preparation for surgery.
  • Provide clear, timely updates to patients and families regarding:
    • Transfer processes
    • Investigation progress
    • MDT outcomes
    • Surgical planning timelines

  • Degree / Diploma qualification in in nursing or health related subject relevant subject
  • Registered Practitioner with current professional registration eg NMC, HCPC
  • Qualification in Teaching and Assessing
  • Thorough and up to date knowledge of nursing theory and best practice particularly within speciality area, including relevant clinical experience

Desirable
  • Evidence of masters level academic study or willingness to work towards Masters (in relevant health related subject
  • Clinical patient assessment module (Such as PACR)
  • Experience of undertaking research or audit projects
