The role acts as a bridge between Rehabilitation Healthcare Support Workers and the Therapy Team. You will spend the first part of the shift supporting ward-based care alongside Rehabilitation Healthcare Support Workers, contributing to personal care and rehabilitative activities, before working with the therapy team later in the shift to deliver prescribed rehabilitation interventions.

You will complete specialist induction and advanced training and will work within clearly defined competencies and under the supervision of qualified therapy staff at all times.

  • Undertake treatment sessions or components of treatment under guidance, following therapy care plans and risk assessments.
  • Feed back to the therapeutic team regarding adjustments to treatment programmes and factors impacting goal attainment

  • Observe, monitor, and accurately report patient progress, tolerance and response to treatment to the supervising therapist.
  • Maintain timely, accurate and confidential clinical documentation in line with Active Neuro standards.
  • Recognise own limitations, maintain professional boundaries and seek advice or escalation when required.

Patient Safety and Quality

  • Deliver care in line with Active Neuro clinical policies, rehabilitation protocols and risk management procedures.
  • Identify and report changes in a patient’s physical, cognitive or emotional condition promptly to the therapy or nursing team.
  • Contribute to infection prevention and control, moving and handling safety, and safeguarding practice.
  • Promote dignity, privacy, equality and patient-centred care at all times.

Team Working and Communication

  • Work collaboratively with physiotherapists, occupational therapists, speech and language therapists, psychologists, nursing staff and support workers.
  • Participate in handovers, therapy planning meetings, multidisciplinary meetings and clinical discussions as required.
  • Communicate effectively with patients, families and carers, providing reassurance and clear explanations within role boundaries.

Administrative and Operational Responsibilities

  • Assist with the efficient running of the therapy service under the direction of a qualified therapist.
  • Prepare treatment areas, equipment and resources for therapy sessions and ensure equipment is cleaned, maintained and stored safely.
  • Report equipment faults, supply shortages and safety concerns promptly.
  • Contribute to audit, service evaluation and quality improvement activities as requested.
  • Support administrative tasks related to therapy delivery and patient scheduling as required.

Professional Accountability and Governance

  • Work within agreed competencies, training and Active Neuro policies at all times.
  • Practice in accordance with Active Neuro’s clinical governance framework and rehabilitation standards.
  • Maintain confidentiality and comply with data protection, safeguarding and information governance requirements.
  • Report incidents, near misses and concerns using Active Neuro reporting systems.

What we would like from you:
