Job summary
An exciting opportunity has arisen for a motivated and enthusiastic individual to join a newly developing Community Dementia Service as a Dementia Specialist Practitioner within the Complex Dementia Team. The Service is currently being established in the Lincoln & Gainsborough area, with plans to expand into a countywide service in the coming years.

The role will involve the assessment, treatment and monitoring of patients with complex dementia presentations to provide support, education and evidence-based interventions to manage complex presentations. As a Specialist Practitioner, the role will also include liaison with stakeholders: primary care colleagues, care home teams, and third-sector agencies.

Main duties of the job
  • To provide a high standard of clinical care, ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.
  • Work with individuals with complex dementia and provide support to them and their carers to understand and manage complex presentations.
  • To delegate aspects of the patient treatment plan to other members of the team whilst maintaining overall responsibility for patients' care, where appropriate.
  • To ensure the clinical caseload and clinical practice of clinical members of the team is of the highest standard of clinical care.
  • To manage caseload and service delivery in accordance with and in order to contribute towards the attainment of Service/Trust productivity requirements.
  • Develop and regularly review care plans and risk assessments.
  • Work in line with evidence-based practice to provide safe and effective care and treatment.
  • Plan, implement, review and improve interventions to meet people's identified needs and manage their inherent risk.
  • To assess carers' and families' needs and develop, implement and review programmes of support for carers and families.
  • Protect people from abuse, neglect

Person Specification
Experience
Essential
  • Relevant experience of working with adults with mental health needs. Or clear evidence of substantial experience of working with individuals in crisis and skills that are transferable to both community/in-patient.
  • Experience of working with people with severe and enduring mental illnesses
  • Experience working as part of a team
  • Experience of being supervised
  • Evidence of continuing professional development

Special requirements
Essential
  • Ability to travel around the county independently without the use of public transport

Skills
Essential
  • Sound knowledge of national agenda for mental health.
  • Highly developed clinical reasoning skills.
  • Sound knowledge of clinical/risk assessment and understanding of Information Governance principles.
  • Highly developed communication skills.

Qualifications
Essential
  • Registered Nurse: 1 st Level Registration (NMC) or degree or equivalent.
  • Currently Registered Nurse
  • Mentorship Module (degree level)
  • Evidence of specialised continued professional training (degree level) in clinical practice.

Desirable
  • Management training
  • ENB998 / FliPS or equivalent
