Quality Commission Job Introduction We have an exciting opportunity for a Band 6 Registered Nurse/ Paramedic to join our Hospital at Home (H@H) Service across Bath and North East Somerset, Swindon, and Wiltshire (BSW). This is an excellent time to join the service as it undergoes a period of transformation and development, creating opportunities to contribute to innovative models of care and service improvement. Working across a variety of community settings and patients homes, you will play a key role in delivering high-quality acute care outside of traditional hospital environments. As part of a multidisciplinary team, you will work collaboratively across acute, community, primary care, and social care services to provide assessment, treatment, and ongoing clinical management for patients requiring hospital-level care at home. The service supports both admission avoidance (step-up) and early supported discharge (step-down) pathways, ensuring patients receive timely care in the most appropriate setting. We are looking for a compassionate and proactive clinician who can work autonomously, make sound clinical decisions, and deliver safe, effective, person-centred care. Main Responsibilities As a Nurse/Paramedic, you will work with a good degree of independence, carrying out tasks and responsibilities delegated by the lead clinician. You will be an important part of the team, helping to meet the needs of patients in community settings, initially focusing on those referred through the 2-hour Urgent Community Response (UCR), and later supporting the development of broader referral pathways. Working within a multidisciplinary team and across primary, secondary, acute, and social care services in Wiltshire, you will play a key role in coordinating specialist interventions for people who might otherwise require hospital care, or who need timely support to leave emergency departments safely. You will take responsibility for your clinical caseload and help ensure smooth coordination of care throughout each patient’s journey. As part of this role, your responsibilities will include: Clinical practice Providing evidence-based advanced clinical skills to support appropriate patient care, under the guidance of a registered clinician. This includes assessing patients, interpreting tests within your scope of practice, and planning care and treatment. You will recognise early signs of deterioration, respond promptly, elevate when needed, and document your actions clearly. Management Organising your time and workload effectively, advocating for patients, managing your caseload day to day, offering advice and treatment recommendations, working closely with clinicians in the Virtual Ward team, and contributing to supervision. Leadership Serving as a positive role model for colleagues and promoting collaborative working. You will help challenge traditional boundaries in a supportive way while upholding high standards of quality, safety, and professional practice. Supporting the learning environment Demonstrating commitment to your own professional development and supporting the learning of colleagues and student nurses. Continuous improvement Noticing areas where practice can be strengthened and contributing to improvement efforts within a Just Culture that values reflection and learning. Ideal Candidate Essential * Registered nurse/Paramedic * Evidence of study at Level 6 or above * Advanced clinical assessment skills * IV therapy competency * Able to work autonomously and as part of a team * Experienced in the use of NEWS2 diagnostic tool and when to elevate * Contribute to the development of others, supervising and teaching, mentoring, including student Nurses * Previous experience in elderly care, frailty, community services or relevent acute care speciality * Excellent written and verbal communication, and interpersonal skills * Ability to adapt to the demands of a constantly changing environment * Good IT skills * Work within the NMC code of conduct. * Knowledge of community environment * Good Communication skills including IT literacy and knowledge of computer systems. * Undertake evidence based clinical practice. * Experience of caring for people with long term conditions * Experience of pressure ulcer prevention * Awareness of self-neglect, safeguarding and abusive situations to be the patients advocate * Ability to be an effective team member.
