Job description
We are seeking to recruit an experienced and compassionate Community Mental Health Nurse to the role of Band 6 within the Older Adults Community Mental Health Team. Alongside the team leader the successful candidate will provide clinical and professional leadership to a community mental health nursing team delivering specialist care to older adults with functional mental illness, dementia and complex co‑morbid physical health needs. The successful candidate will support delivery of high quality assessment, risk management and person‑centred interventions within community settings. You will maintain a caseload of complex service users, support staff wellbeing and development, and contribute to service improvement and clinical governance activity. The role involves close partnership working with carers, primary care, social work, inpatient services and third‑sector partners, ensuring safe, effective and coordinated care across the Older Adults pathway. Applicants must be Registered Mental Health Nurses with significant post‑registration experience and evidence of leadership or supervisory experience within mental health services. Driver and access to a car essential to role.
