Job description

We are looking for a Rapid Access to discharge (RADS) Nurse Clinician to join one of our specialist Teams Working within the Hospice Community Services Team, as part of the hospice rapid access service, this role undertake holistic assessments and support patients who require palliative and end of life care and are rapidly deteriorating. You will be assessing their needs in order to organise personal care with Domiciliary care providers in their home or sourcing 24 hr care provision working with them and families/carers. As professionals in the field of specialist palliative and end of life care, our aim is to work collaboratively for patients and their families/carers facing death and bereavement. The role of RAD’s Clinician is instrumental in ensuring that St Luke’s Hospice meets the increasing demand from its community. The extensive multidisciplinary team (MDT) will coordinate and deliver holistic care to patients at all stages of end of life. The team will also provide essential support to carers and family members. The Rapid Access to discharge team exists to help people who are nearing end of life to achieve their preferred place of care and support people’s choices regarding where they wish to die. The team works with others teams in the hospice and external partners. In this role you will: · Work within a multi-disciplinary team providing clinical care, navigation, coordination and support to hospice patients and their families and carers for patients who are thought to be in the last three months of life. · Work collaboratively as part of the multi-disciplinary team (MDT) sharing the clinical caseload of patients/carers within the hospice community services, patient and family support services. · Use holistic assessment to provide information, advice, psychosocial and spiritual care to patients with palliative and end of life care needs, supporting their families and carers, liaising and referring on as necessary to appropriate health, social and voluntary care professionals to enable them to receive care in the location preferred at the end of their lives. · Work in partnership with those receiving care, helping them to access relevant health and social care information, voluntary care services and support when they need it. Support people to access, identify, appraise and interpret information, to enable them to determine their own future. · Coordinate and negotiate placements with care homes and/or domiciliary care providers for home, residential home and oversee ongoing monitoring and review of such placements in accordance with the Hospice service specifications. · Be empathetic with excellent communication skills with a passion for service improvement within healthcare. You will participate in building and maintaining strong relationships with external partners and stakeholders to deliver the best possible coordinated care for patients. · This role is supported by the Hospice Rapid Access Team Leader and will have the opportunity to develop and improve their skills. The successful candidate will: Be a Registered Nurse Have significant experience in the delivery of end of life and palliative care or related disciplines Have excellent IT, organisation and communication skills Be a car driver with vehicle available for work purposes Salary : £32,632.13 to £41,563.99 (Full Time Equivalent) per year (depending on experience). The pro rata salary for this role based on 15 hours will be £13,052.85 to £16,625.60 per year (depending on experience) Contract : Permanent (to start as soon as possible) Hours : 15 hours over 2 days per week (weekends). There may also be the opportunity to undertake additional shifts as the service operates 7 days per week.
