Make a Real Impact at the Heart of Adult Community Services

We are looking for an experienced, motivated, and people-focused Senior Community Health Navigator to join our BSW (Bath & North East Somerset, Swindon & Wiltshire) Adult Community Services team.

This is an exciting opportunity to take on a leadership role within a busy and dynamic service, supporting the day-to-day operational management of our Community Health Navigator and Administration Team. You will play a key role in ensuring the delivery of a high-quality, responsive service that supports patients, colleagues, and partners across Adult Community Services.

If you are an organised and proactive individual with excellent communication skills and a passion for supporting others, we'd love to hear from you.

Why apply?
This is a great opportunity to develop your leadership skills, gain valuable management experience, and make a meaningful difference to the delivery of adult community services. You’ll be part of a supportive team, with the chance to shape how the service operates and further your career progression within the organisation.

Base

The role is primarily based at Jenner House, Chippenham, Wiltshire, SN15 1GG, and operates on a hybrid working model. You will be required to travel across the BSW area as part of the role.

Please note: Applications from candidates who have previously applied for this role will not be considered and should not reapply.

What You'll Be Doing

  • Oversee the day‑to‑day management of the Adult Administrator and Community Health Navigator SPA workforce, ensuring effective rotas are in place and adapted as needed to maintain full service cover
  • Work in line with guidelines and protocols, collaborating closely with clinical colleagues to support safe and effective service delivery
  • Develop, review and maintain robust processes for the adult administration function to support operational efficiency
  • Lead on the recruitment of administration staff, including shortlisting, interviewing, induction and ongoing training
  • Maintain oversight of HealthRoster, ensuring accurate completion and booking of bank shifts where required
  • Manage telephony performance, referral management and scheduling of clinical appointments on SystmOne, ensuring patients are seen within contracted waiting times
  • Provide direct line management to the administration team in line with HR policies, including one‑to‑ones, appraisals, performance management and staff development
  • Ensure clear, timely communication across the administration team and maintain strong working relationships with clinicians and managers
  • Contribute to BSW Adult SPA team meetings, Datix investigations and the embedding of learning into team processes
  • Prepare and summarise data to support reporting and service improvement
  • Act as a friendly, professional first point of contact for service users, handling sensitive and confidential information with empathy and discretion
  • Work flexibly and provide cover for colleagues when required, undertaking additional duties as delegated by the line manager

Please see job description attached for full details of the role.

About the Company

We change lives by transforming health and care.

Established in 2006, we are one of the UK's leading independent providers of community health and care services, working with health and care commissioners and communities to transform services with a focus on experience, efficiency and improved outcomes. We deliver and transform adult and children community health services, primary care services including urgent care, sexual health, dermatology and MSK services as well as adult social care and wellbeing services. Across England, we support communities of many millions and directly help more than half a million people each year - guided by our simple values: we care, we think, we do.

We're committed to equal opportunities and welcome applications from a broad, diverse range of people who want to join our team. We’re a Disability Confident Committed company, so we work to provide facilities, work environment adjustments and technical solutions to be as inclusive of everyone.

While it doesn’t happen often, sometimes a role is very popular, and we’ll need to close it earlier than the date we’ve shown here. If you’re keen to join our team, we’d love to hear from you so please apply as soon as you can.

As you’d expect, safeguarding and protecting the children, young people and vulnerable adults that we work with is of the utmost importance so we have policies and procedures in place to promote safeguarding and safer working practices and everyone who joins the team is subject to a safer recruitment process, including the disclosure of criminal records and vetting checks.

Finally, we need to let you know that the company you’ll work for is part of HCRG Care Group Holdings Limited and by applying for this job we’ll need to process and hold information about you. If you would like to know a little more about how we use your information, please see our website's privacy policy.
