Job summary
Easington Central PCN is seeking an enthusiastic and patient-focused Clinical Pharmacist (NMP preferred) to join our multidisciplinary team, primarily aligned with Horden Group Practice. This is a three day (Mon, Tues & Fri), part time position (22.5hrs). This is an exciting opportunity to play a key role in delivering high-quality, safe, and effective medicines optimisation services to our patient population.

The successful candidate will possess excellent communication and interpersonal skills, with the ability to build strong working relationships with patients, general practice teams, community pharmacists, and partner organisations. You will be motivated, organised, and able to effectively manage competing priorities while contributing to the achievement of key clinical, quality, and operational targets.

Main duties of the job
Working within your professional competence and scope of practice, you will provide a clinical pharmacy service as part of a multidisciplinary team within Easington Central PCN and Horden Group Practice. You will support the delivery of high-quality patient care through medicines optimisation, safe prescribing practices, and effective management of medicines across care settings.

The post holder will undertake patient-facing structured medication reviews, with a particular focus on patients with long-term conditions, polypharmacy, frailty, and complex health needs. You will work proactively to identify and address medicines-related risks, improve prescribing quality, and support patients to achieve the best possible outcomes from their treatment.

You will work collaboratively with GP practices, community pharmacy colleagues, secondary care providers, and other healthcare partners to ensure safe and seamless transfer of care, including medicines reconciliation following hospital discharge.

The role will contribute to the delivery of national and local medicines optimisation priorities, supporting quality improvement initiatives, prescribing audits, medication safety programmes, antimicrobial stewardship, and the achievement of PCN and practice objectives. The post holder will also play a key role in ensuring the cost-effective use of medicines while maintaining the highest standards of patient safety and clinical effectiveness.

About us
In this role, you will support the safe, effective, and cost-efficient use of medicines, helping to improve prescribing quality, reduce medication-related risks, and enhance patient outcomes. You will undertake structured medication reviews, support medicines reconciliation following discharge from hospital, and provide expert pharmaceutical advice to clinicians and patients.

Alongside patient-facing clinical responsibilities, you will contribute to practice and PCN performance through medicines optimisation initiatives, prescribing quality improvement programmes, clinical audits, and the delivery of key contractual requirements, including Quality and Outcomes Framework (QOF) indicators and medicines safety objectives.

Experience within primary care is desirable but not essential. We welcome applications from pharmacists with a passion for patient care, a proactive approach to learning, and a commitment to collaborative working. The ability to work effectively as part of a team, demonstrate flexibility, and prioritise workload is essential.

Qualifications
Essential
  • Registered with the General Pharmaceutical Council (GPhC)
  • Degree in Pharmacy (MPharm) or equivalent
  • Specialist knowledge acquired through postgraduate diploma-level study or equivalent training/experience

Desirable
  • Membership of the Royal Pharmaceutical Society (RPS)
  • Member of, or working towards, Faculty Membership of the Royal Pharmaceutical Society
  • Independent Prescriber qualification or working towards / committed to obtaining qualification
  • CPPE Primary Care Pathway completed or working towards completion
