Job summary
We are looking for a Medical Nurse Practitioner or Advanced Care Practitioner with a passion for Frailty, to join our new Frailty Same Day Emergency Care (SDEC) Service and OPU team.

This is an exciting development for the RUH, creating a unique opportunity to make real change and impact at the front door.

You will work closely with consultants, advanced clinical practitioners, nursing and therapy colleagues in our new Frailty Same Day Emergency Care Service, where we link in with the Emergency Department to deliver multi-disciplinary, patient-centred care, advising on discharge or admission with the complex caseload this interesting group provides.

You will be supported with teaching and development skills and opportunities to build your own knowledge and skills, helping to shape your future pathways in this area, as we move towards an integrated front door.

Main duties of the job
To hold responsibility for own case load and provide a high standard of assessment, diagnosis and planning for:
  • Frailty - a dynamic team of geriatricians, medical nurse practitioners and occupational therapists and physiotherapists providing early assessment and intervention in FSDEC and the emergency department to those patients living with frailty.
  • Linking in with the Older People Unit Short Stay (OPUSS) ward and frailty assessment areas which are our short-stay, older people's inpatient ward for our patients who require a bed for less than 72 hours.
  • Core wards - Combe and Waterhouse
  • Provide clinical support, teaching and education to colleagues and families
  • Support with future planning and management of Frail patients
  • Represent MNPs and work with other members of MDT to support service improvement and best practice.

Qualifications and Training
Essential
  • Registered Nurse Part 1, 1,12
  • Registered with the Nursing and Midwifery Council
  • Relevant clinical supervision training and experience
  • Registered Non-Medical Prescriber

Desirable
  • ILS/ALS course
  • Recognised leadership course

Knowledge and Experience
Essential
  • Minimum one years' experience as a Nurse Practitioner at Band 7
  • Evidence of autonomous practice Management of Patients with complex needs
  • Knowledge of local and national health policy
  • Ability to implement and manage change effectively
  • Knowledge of Clinical governance issues in secondary care
  • Evidence of CPD relevant to role

Desirable
  • Clinical leadership experience
  • Evidence of research involvement

Phsical skills and emotional effort
Essential
  • Able to work and cope with rotating shift patterns over 7 days of the week.
  • Able to sensitively communicate with patients/relatives and staff on emotional and complex issues such as: - diagnosis and treatment - ceiling of treatment - end of life - complaints; investigation and resolution
  • Conflict - Ability to deal with conflict in the clinical environment Ability to challenge practice, lead others and justify changes
  • Dexterity to undertake specific clinical procedures

Working Environment
Essential
  • Involves direct contact with body fluids, products, blood, sputum and vomit, foul linen etc on a number of occasions per shift
  • Involves risk of verbal and physical aggression from patients and relatives
