Job description

Job summary
We are currently seeking a Community sister (band 6) to join our Community Nursing Team for Durham locality based at Chester le street. The team cover 8am until 9pm 7 days a week on a duty roster. The role involves providing holistic nursing care to adult patients in their own home or care home settings. Successful applicants will be required to hold a first level nursing registration with the NMC, additional training for non-District nurses will be via a 2 year apprenticeship subject to next available funding.

Main duties of the job
The Community nursing team provide both planned and reactive care including Urgent Community Response. As a district nurse/community sister you will directly manage staff aligned to you, delivering hands on care and supporting the team with training and senior clinical knowledge.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities
The successful candidate will provide a variety of nursing care to patients, including wound care, administration of medication, palliative care, management of bowel and catheter care. The role involves assessment of patient care needs, care planning, delivery of care and review. The successful candidate will be required to work independently as part of the wider Nursing team reporting back to team leads. this role has supervision and management of staff along with co-ordination shifts and triage duties.
