Job description

Job summary
We are looking for an experienced, enthusiastic and compassionate Practice Nurse to join our friendly and supportive team at Park View Medical Centre. This is an exciting opportunity for an NMC-registered nurse with previous GP Practice Nurse experience to work within a busy, high-achieving training practice caring for approximately 6,000 patients. The successful candidate will provide high-quality, evidence-based nursing care, managing their own clinics and supporting patients with long-term conditions while working collaboratively with our multidisciplinary team. The role includes chronic disease management (including diabetes, asthma and COPD), cervical cytology, immunisations, travel vaccinations, contraception, health promotion and other core practice nursing duties. Please note: This vacancy is only suitable for applicants with current or previous GP Practice Nurse experience. Applications from nurses without at least 2 years General Practice experience and recruitment agencies will not be considered.

Main duties of the job
The successful candidate will work independently within their professional scope of practice to deliver high-quality, evidence-based nursing care to our practice population.
Main duties include:

  • Managing chronic disease clinics, including diabetes, asthma and COPD.
  • Undertaking cervical cytology (smear tests).
  • Administering routine, childhood and travel immunisations and vaccinations.
  • Providing contraception advice and associated services.
  • Carrying out venepuncture, ECGs, spirometry, peak flow measurements and other clinical procedures.
  • Delivering NHS Health Checks, health reviews and health promotion advice.
  • Supporting and promoting infection prevention and control by adhering to national guidance and practice policies, maintaining safe clinical environments, ensuring appropriate vaccine storage and stock management, and contributing to infection control audits and quality improvement activities.
  • Maintaining accurate clinical records using the practice clinical system.
  • Working collaboratively with GPs, the wider multidisciplinary team and external healthcare professionals to provide safe, effective patient care.
  • Participating in audits, clinical governance, mandatory training and continuing professional development.
  • Contributing to the achievement of CQC standards and contractual quality requirements.

Job responsibilities
The post holder will be responsible for:

  • Delivering high-quality nursing care within their scope of practice.
  • Managing long-term condition clinics, including diabetes, asthma and COPD.
  • Undertaking cervical screening (smear tests).
  • Administering routine, childhood, travel and seasonal immunisations and vaccinations in accordance with national and local guidance.
  • Providing contraception advice and associated services within competence.
  • Performing wound care, dressings, suture and clip removal.
  • Undertaking venepuncture, ECGs, spirometry, peak flow measurements, urine testing and other clinical procedures.
  • Carrying out NHS Health Checks, cardiovascular risk assessments (including QRISK) and health promotion activities.
  • Administering prescribed injectable medications and participating in anaphylaxis and resuscitation procedures.
  • Making appropriate referrals to GPs and other members of the multidisciplinary team.
  • Maintaining accurate, timely and confidential electronic patient records using the practice clinical system.
  • Ensuring specimens are correctly labelled, stored and dispatched to the laboratory.
  • Promoting healthy lifestyles and supporting patients to self-manage long-term conditions.


Infection Prevention and Control
The post holder will promote and maintain high standards of infection prevention and control by:

  • Adhering to national guidance, practice policies and standard infection prevention procedures.
  • Maintaining clean, safe clinical environments.
  • Ensuring the safe storage, handling, rotation and disposal of vaccines, medicines and clinical equipment.
  • Reporting and escalating infection control concerns where appropriate.
  • Participating in infection prevention audits and quality improvement activities.


Professional Responsibilities
The post holder will:

  • Maintain active NMC registration and comply with the NMC Code.
  • Undertake mandatory training and continuing professional development.
  • Work within practice policies, procedures and clinical protocols.
  • Recognise own limitations and seek advice or refer appropriately.
  • Participate in clinical audit, quality improvement and service development.
  • Support students and other learners where appropriate.
  • Contribute to the achievement of CQC standards and contractual requirements.


Team Working
The Practice Nurse will work collaboratively with GPs, nurses, healthcare assistants, pharmacists, administrative staff and external healthcare professionals to ensure the delivery of safe, effective and compassionate care. Flexibility is expected to support colleagues and meet the needs of the service.

Confidentiality, Health & Safety and Safeguarding
The post holder will:

  • Maintain patient confidentiality and comply with UK GDPR and Data Protection legislation.
  • Safeguard children and vulnerable adults in accordance with national guidance and practice policies.
  • Comply with all Health and Safety legislation and practice procedures.
  • Report incidents, accidents and hazards appropriately.

Essential

  • Previous experience working as a GP Practice Nurse.
  • Experience independently managing long-term condition clinics, including diabetes, asthma and COPD.
  • Experience undertaking cervical cytology.
  • Experience delivering routine, childhood, travel and seasonal immunisations and vaccinations.
  • Experience in wound care, dressings and suture removal.
  • Experience undertaking venepuncture, ECGs, spirometry and other routine clinical procedures.
  • Experience using electronic patient record systems within General Practice.
  • Experience working effectively as part of a multidisciplinary team.
  • Experience promoting and maintaining infection prevention and control standards within a clinical setting.


Desirable

  • Two or more years' experience as a GP Practice Nurse.
  • Experience supporting QOF and long-term condition reviews.
  • Experience participating in clinical audit and quality improvement activities.


Qualifications
Essential

  • Registered Adult Nurse with current NMC registration.
  • Nursing Diploma or Degree.
  • Evidence of continuing professional development.
  • Cervical Screening (Smear Taker) qualification or experience.
  • Immunisation and vaccination training.
  • Training and competency in the management of long-term conditions, including diabetes, asthma and COPD.


Desirable

  • Additional qualifications relevant to General Practice nursing.
