Job Summary

We have an exciting opportunity for a motivated and forward-thinking Clinical Lead Nurse to join our highly valued Care Home Intensive Support Team (CHIST). CHIST is a dynamic, responsive service that brings together Nursing, OT, and Speech and Language Therapy professionals to provide targeted, specialist support to care homes across Staffordshire. Our mission is simple: to improve health outcomes, enhance quality of care, and help residents live healthier, more fulfilling lives while avoiding unnecessary hospital admissions. As a Clinical Lead Nurse, you'll be at the forefront of supporting care homes through hands-on clinical interventions, expert advice, and collaborative working. Main Duties of the Job

  • Work closely with health and social care partners to ensure coordinated, seamless care.
  • Identify risks early, prevent deterioration and help reduce avoidable hospital admissions.
  • Promote high standards of care through evidence-based practice and clinical leadership.

Person Specification
Qualifications & Training - Essential

  • Degree in nursing or equivalent
  • Registration with NMC
  • Post Graduate studies in relevant nursing field such as end of life care, long term conditions/palliative care or related/relevant subject
  • Mentorship qualification (998-997)


Qualifications & Training - Desirable

  • Modules at Masters level
  • Evidence of further clinical study


Experience - Essential

  • Relevant experience to the role including knowledge and management of long term conditions
  • Ability to manage time and workload of both the team & self effectively
  • Working in a multidisciplinary team
  • To have knowledge & experience working within both nursing & residential care setting


Experience - Desirable

  • To be able to negotiate and manage conflict in clinical situations
  • To be able to lead and supporting staff
  • To be able support in service improvement


Skills, Knowledge & Abilities - Essential

  • Evidence of specialist clinical knowledge relevant to the post
  • Understanding of Information Governance, Data Protection, NMC standards and Safeguarding Vulnerable Adults
  • To have highly specialised clinical skills in a wide range of nursing knowledge to enhance the health & wellbeing of care home residents
  • Awareness of current guidance in relation to Covid 19, outbreaks and infection prevention
  • & have an ability to prioritise the support needed in a care home & escalate any issues appropriately
  • Awareness of relevant national policy and guidance
  • Basic IT skills
  • To be committed to evidence practice, to have excellent clinical leadership skills & be willing to take on additional responsibilities essential to the ongoing development of the team


Skills, Knowledge & Abilities - Desirable

  • Understanding of wider NHS agenda and its application to the local context
  • Understanding/knowledge of the governance & regulatory frameworks pertinent to care homes
