St Barnabas House provides specialist palliative care to adults in the Worthing, Adur, Arun and Henfield areas through a range of services, including in-patient care and community living well services.

Job responsibilities
  • To work as part of the Community Team, working collaboratively with the wider healthcare teams.
  • To provide expert palliative care to support the complex needs of patients with progressive life limiting illnesses.
  • To develop knowledge and skills in the provision of high quality, research based supportive and palliative care to patients with progressive life-limiting illnesses.
  • Provide clinical advice and expertise to help guide the primary care teams to anticipate potential problems, encourage and support advance planning, to recognise and respond appropriately to palliative care emergencies.
  • Within own level of competence, advise other health professionals on symptom management and care planning (including social and psychological care) which promotes evidence-based practice and maximises the patient's potential outcome.
  • To advocate for patients and their families, providing information which will allow for informed discussions, enabling choices to be made concerning treatments and/or future care.
  • Use interpersonal skills to advise and inform patients and their families, primary healthcare team and colleagues regarding pain and symptom management, psychological, social, spiritual support and bereavement care.
  • Complete patient assessments using a range of clinical skills, knowledge and expertise and ensure the full range of MDT resources are offered to meet individual patient and carers needs.
  • To provide telephone clinical support and advice to patients, their families and health care professionals via the telephone advice line known as the Hub which may involve working unsocial hours.
  • To demonstrate advanced communication skills with patients, families, carers and other health care professionals, in complex and emotionally distressing situations, to facilitate holistic palliative care.
  • To identify specific educational needs of individual patients, carers and health care professionals.
  • To implement user involvement initiatives to create a more patient responsive service.
  • To participate in and contribute to research and audit and incorporate evidence based knowledge into-day to day practice, in line with national and local initiatives and guidance.
  • To increase knowledge of national policy initiatives in palliative care to play an active role in their local dissemination.
  • To maintain a high professional standard within own professional practice and development in accordance with NMC standards.
  • Attend relevant meetings such as Gold Standard Framework (GSF) meetings, MDTs and capacity meetings to determine plan of care and appropriate actions for patients and their families.
  • To have a good working knowledge of information management and technology, including but not limited to, email; electronic patient record system; word documents.
  • To enter clinical data accurately to support team and service development and ensure patient centred care.
  • To be involved in clinical audit contributing to the Hospices overall clinical governance policy.

Person Specification
Qualifications
Essential
  • Significant recent post registration experience, at AFC Band 5 or above in relevant area
  • Current professional qualification and NMC registration/RN level 1
  • Degree level qualification in cancer/palliative care or relevant subject or working towards
