Job description

Job Introduction
As a Band 5 Community Staff Nurse, you will join our valued Community Nursing Team based in Gravesend, North Kent. This is a rewarding opportunity to deliver care where it matters most, in patients own homes and local communities. You will support individuals with a wide range of healthcare needs, helping them to manage long-term conditions, recover from illness and maintain their independence while avoiding unnecessary hospital admissions. Community nursing offers variety, autonomy and the chance to build meaningful relationships with patients and families. No two days are the same, and you will have the opportunity to develop a broad range of clinical skills while making a genuine difference to the lives of people within your local community. Whether you are newly qualified or already have nursing experience, you will be supported to grow and develop within a friendly, welcoming and collaborative team environment. Newly qualified nurses will also benefit from our award-winning Preceptorship Programme, designed to support your transition into community practice. A full UK driving licence and access to a vehicle for work purposes is essential.

Package Description
You will be joining a supportive and experienced community nursing team who are passionate about delivering high quality, patient-centred care. The team works closely together, sharing knowledge and supporting one another to ensure the best outcomes for patients. The service operates seven days a week between 8:00am and 8:00pm.

Main Responsibilities
As a Community Staff Nurse, you will assess, plan, deliver and evaluate nursing care for patients within their own homes and community settings. Working closely with Senior Sisters, GPs, therapists and wider multidisciplinary colleagues, you will help ensure patients receive safe, effective and compassionate care tailored to their individual needs. You will manage and prioritise your own workload, responding to changing clinical needs and ensuring patients and carers remain fully involved in decisions about their care. You will also play an active role in health promotion, supporting people to improve their wellbeing and maintain their independence wherever possible. Alongside your clinical responsibilities, you will contribute to service improvement initiatives, clinical audits and the ongoing development of community nursing services, helping us continue to deliver high quality care across Gravesend. Key responsibilities include:

  • Delivering high quality nursing care to patients within their homes and community settings
  • Assessing, planning, implementing and evaluating patient care
  • Monitoring patients conditions and escalating concerns appropriately
  • Working collaboratively with patients, carers and multidisciplinary teams
  • Maintaining accurate and timely patient records using electronic systems
  • Promoting health, wellbeing and independence
  • Participating in team meetings, clinical supervision and professional development activities
  • Supporting evidence-based practice, quality improvement and service development initiatives


Person Specification
Essential

  • Registered Nurse with a valid NMC registration
  • Evidence of continuing professional development
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Strong organisational and prioritisation skills
  • Commitment to delivering high quality, patient-centred care
  • Confidence using IT systems, including Microsoft Office applications
  • Full UK driving licence and access to a vehicle for work purposes


Desirable

  • Previous experience within a community, primary care or district nursing setting
  • Experience supporting patients with long-term conditions or complex healthcare needs
  • Mentorship or teaching qualification, or willingness to undertake further development
  • Knowledge of current developments in community nursing and integrated healthcare services
