Job description
Job Introduction
As a Band 6 Team Sister, you will play a key role within our Community Nursing Service, providing both clinical and operational leadership to support patients with complex health needs within their own homes and local communities. This is an exciting opportunity for an experienced and motivated nurse who is passionate about delivering high-quality community care. You will manage a varied caseload, working with patients requiring support with wound care, long-term conditions, end of life care and complex clinical interventions. Using your clinical expertise and leadership skills, you will help ensure patients receive the right care, at the right time, from the right professional. You will work closely with the Senior Sister and wider multidisciplinary team, supporting colleagues, driving service improvements and helping to develop a culture of continuous learning and high-quality patient care.

Main Responsibilities
As a Team Sister, you will manage a defined nursing caseload, delivering and coordinating care for patients with a range of complex and long-term health conditions. You will undertake comprehensive assessments, develop personalised care plans and ensure care is regularly reviewed to meet changing patient needs. You will provide clinical leadership within the team, supporting the Senior Sister with service delivery, triage processes, caseload management and workforce development. You will act as a role model for best practice, supporting colleagues through mentorship, supervision and clinical guidance while promoting a positive and supportive team culture. Working closely with patients, carers, GPs, specialist teams, social care professionals and partner organisations, you will help ensure seamless and coordinated care that improves patient outcomes and promotes independence. Key responsibilities include:
  • Managing a complex community nursing caseload
  • Delivering high-quality clinical assessment, care planning and evaluation
  • Supporting patients with long-term conditions, frailty and complex health needs
  • Delivering and coordinating interventions including wound care, end of life care and unscheduled care
  • Supporting admission avoidance and promoting patient independence
  • Participating in referral triage and caseload allocation
  • Supervising, supporting and developing junior nursing colleagues
  • Contributing to clinical governance, audit and service improvement initiatives
  • Building strong relationships with multidisciplinary and partner organisations
  • Maintaining accurate and timely patient records

Person Specification
Essential
  • Registered Nurse with current NMC registration
  • Evidence of post-registration professional development
  • Experience working within community nursing, district nursing or a similar clinical setting
  • Experience managing patients with complex health needs and long-term conditions
  • Experience supervising, mentoring or supporting other staff members
  • Excellent clinical assessment and decision-making skills
  • Strong communication, leadership and interpersonal skills
  • Ability to work autonomously and effectively manage a varied workload
  • Excellent organisational and time management skills
  • Good IT skills, including electronic patient record systems and Microsoft Office
  • Full UK driving licence and access to a vehicle for work purposes

Desirable
  • Degree level qualification or working towards one
  • Teaching, mentorship or assessing qualification
  • Independent Prescribing qualification or willingness to undertake further development
  • Experience conducting appraisals, clinical supervision or leadership responsibilities
  • Experience contributing to service development, quality improvement or audit projects
  • Knowledge of community nursing pathways, clinical governance and integrated care services
