Our integrated Stroke Therapy Service provides specialist rehabilitation across the entire stroke pathway, including the Hyper Acute Stroke Unit (HASU), Acute Stroke Unit, and Enhanced Early Supported Discharge (ESD) Service. This enables us to support patients from the earliest stages of recovery through to rehabilitation within their own homes, delivering high-quality, patient-centred care closer to home. Based at Royal Stoke University Hospital, this role offers a unique opportunity to work across both inpatient and community settings, providing assessment, treatment and rehabilitation for patients with complex stroke-related impairments. The successful candidate will be expected to undertake community visits and therapy reviews as part of the ESD pathway. Interdisciplinary working is central to our practice, and you will work closely with colleagues across physiotherapy, occupational therapy, speech and language therapy, nursing and medical teams to support patients in achieving their rehabilitation goals and maximising independence. A full UK driving licence and access to a vehicle for work purposes are essential requirements for this role due to the community element of the service. Main duties of the job We are seeking to appoint an enthusiastic and forward-thinking Physiotherapist with an interest in developing advanced rehabilitation skills across both the acute stroke pathway and Early Supported Discharge (ESD). You will be an integral member of our integrated Stroke Therapy Service, working within a 7-day service model (07:30–20:00) across the Hyper Acute Stroke Unit (HASU), Acute Stroke Unit, and community-based rehabilitation services. The post requires excellent interpersonal and communication skills, enabling effective multidisciplinary working across physiotherapy, occupational therapy, speech and language therapy, nursing, and medical teams. You will play a key role in facilitating early rehabilitation for patients following stroke and other neurological conditions, supporting the development and delivery of goal-focused treatment plans within an interdisciplinary framework. A key aspect of the role is promoting a seamless and timely transition from hospital to home, ensuring continuity of rehabilitation through the ESD/TED pathway. You will support patients to continue working towards their rehabilitation goals within their own home or usual place of residence, with a focus on maximising independence and functional recovery. You will be expected to manage your own caseload autonomously, prioritise workload effectively, and work independently across both inpatient and community settings. Person specification Qualifications * Degree/ Diploma in Physiotherapy * Evidence of CPD * Registered with HCPC * Stroke specific trainings * CPD relevant to speciality area Skills * Ability to communicate effectively and build effective working relationships * Demonstrate effective clinical skills * Knowledge of interdisciplinary working * UK issued driving license & access to a vehicle for work purposes * Ability to manage complex patients with multiple co-morbidities * Experience contributing to service development, audits or quality improvement projects * Ability to reflect on own skills and areas for development * Evidenced based knowledge and understanding of Therapy standards and treatment approaches * Experience supervising students, assistants or junior staff and be a clinical Educator Experience * Experience working within the NHS as a qualified therapist * Experience gained in the acute setting as a qualified therapist * Experience of working within Neurosciences/Stroke * Experience of managing patients with complex neurological presentations, including stroke, acquired brain injury or progressive neurological condition * Experience contributing to service development, audits or quality improvement projects * Experience gained in the community setting as a student or qualified therapist * Experience of working within Trauma & Orthopaedic Knowledge * Knowledge of current stroke rehabilitation guidelines and pathways * Understanding of neuroplasticity and evidence-based stroke rehabilitation * Understanding of integrated discharge and community stroke pathways * Knowledge of spasticity, contracture prevention and postural management * Knowledge of cognitive, perceptual and communication impairments and their impact on rehabilitation * Understanding of goal setting and person-centred rehabilitation principles We are committed to equality of opportunity and welcome applications from everyone regardless of ethnicity, disability, gender, age, faith or sexual orientation.
