We're looking for an experienced Community Palliative Care Clinical Nurse Specialist to join our service in a lead role. This is a unique opportunity to remain clinically focused, providing advanced assessment, symptom management and independent prescribing for patients with complex palliative and end of life care needs, whilst also undertaking day-to-day leadership and line management responsibilities within our specialist team.

Main duties of the job
The Community Specialist Palliative Care Team provides expert assessment, symptom management, care planning and emotional support for patients with complex palliative and end of life care needs across our neighbourhood teams in the Doncaster community. The team works closely with primary care, community services, acute providers and voluntary sector partners to ensure patients and families receive coordinated, high-quality care in their preferred place.

As the Community Palliative Care Clinical Nurse Specialist Team Lead, you will continue to deliver expert specialist palliative care through autonomous assessment, symptom management, independent prescribing and ongoing support for patients, families and carers, while also providing day-to-day line management and professional support to a designated group of staff. Approximately 80% of the role is dedicated to direct clinical care, with 20% focused on leadership, staff support and service improvement.

This is an exciting opportunity for an experienced Clinical Nurse Specialist who is passionate about delivering exceptional patient care while supporting colleagues, contributing to service improvement and helping to shape the future delivery of specialist palliative care across our neighbourhood teams.

Essential

  • NMC Registered Nurse (adult)
  • Independent Non-Medical Prescriber
  • Post-registration qualification in Palliative and End of Life Care or demonstrable experience knowledge and clinical expertise in palliative care and symptom management


Desirable

  • Teaching/assessing qualification


Experience
Essential

  • Significant post-registration experience in specialist palliative and end of life care
  • Experience working as a Clinical Nurse Specialist or in an equivalent specialist role
  • Experience of autonomous assessment, management and coordination of patient with complex palliative care needs
  • Experience of working collaboratively within multidisciplinary and multiagency teams
  • Experience of line management, including supervision, appraisal and staff development
  • Experience of contributing to service improvement and quality improvement initiatives


Desirable

  • Experience working within community palliative care services
  • Experience working within a hospice setting
  • Experience of managing performance, attendance and employee relations matters
  • Experience of audit, service evaluation or quality improvements projects
  • Experience of resource or budget management

  • Specialist knowledge of palliative and end of life care, including the assessment and management of complex symptoms
  • Knowledge of current evidence-based practice, national guidance and best practice relating to palliative and end of life care
  • Knowledge of safeguarding adults and children and associated legislation
  • Understanding of clinical governance, risk management ad quality improvement
  • Knowledge of multidisciplinary and integrated models of care
