Full-Time Somerset & Union Counties NJ
BAYADA Home Health Care is seeking an experienced and compassionate Licensed Medical Social Worker (LSW/LCSW) to join our Union County Visits team. This full-time position provides one-on-one home health visits to adult and geriatric clients throughout Somerset and Union Counties NJ.
As a Medical Social Worker youll play a vital role on an interdisciplinary home health team helping clients and their families navigate the social emotional and environmental challenges that impact their health and well-being. Your expertise will connect clients with community resources provide advocacy and help them safely remain in the place they call home.
Must hold an active New Jersey Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) license.
Home health experience is preferred but not required.

Base Pay depending upon qualifications status and subject to negotiation:
$60.00 - $70.00 per point
