The Older Adult Hospital at Home Team (OAHaHT) is a new innovative service in the Black Country which delivers intensive, multidisciplinary homebased care to older adults experiencing acute mental health deterioration, aiming to avoid unnecessary hospital admission, facilitate timely discharge, and provide high quality crisis intervention. The team works across community, care home, domiciliary and outpatient pathways and interfaces closely with inpatient wards, CMHTs, liaison services, and primary care.
Main Duties of the JobThe OAHaHT model aligns with national drivers including the NHS Long Term Plan, NICE guidance, Ageing Well initiatives, and the Crisis Resolution/Home Treatment Team Fidelity Model. The team provides care from 08:00-20:30, seven days per week. The two Consultants will function as key medical members and leaders of the OAHaHT, supporting assessment, formulation, crisis planning, clinical reviews, and MDT decision making. The team is expected to maintain a caseload of between 30-40 patients across the 4 localities.
About UsOlder Adult Mental Health Services deliver multidisciplinary inpatient and community care for older adults with mental health conditions, underpinned by a person-centred and holistic model of care. Services operate across all four Black Country localities, with inpatient provision based in Walsall at the Dorothy Pattison Hospital Older Adult Unit, and at the newly refurbished Edward Street Hospital. These units provide specialist care for older adults with a range of mental health conditions, including functional illness and dementia. Exciting new developments include the Older Adult Hospital at Home Team and In-Reach to Care Home Team, enabling assessment and treatment in community settings and supporting admission avoidance where appropriate.
