Clinical Pharmacist - Frailty

Job Summary
Fixed Term Contract until 31st March 2028. Up to 5 Sessions per week.

Hampshire and IOW (HIOW) Integrated Care Board (ICB) have recently set out our high-level ambitions by describing the strategic vision for primary and local care transformation over the next three years. Hampshire & Isle of Wights population is ageing, with the 90 plus age group expected to increase by 93 per cent by 2043. As frailty and multimorbidity rise, there is a clear need for Integrated Neighbourhood Working (INW) to support independence and reduce utilisation of urgent and acute services within neighbourhoods and hospital settings. Gosport Central PCN are looking for a qualified clinical pharmacist to provide structured medication reviews for this patient group. This role will support in completing the structured medication reviews for these patients and support the team in the multidisciplinary team approach in preventing admission to hospital and supporting patients in their independence in the community.

You will be a key part of a team consisting of a Nurse, Pharmacy Technician, OT, Physiotherapist, Paramedic and GP Assistant to help deliver the aims and objective of the scheme. This is a great opportunity to join the Gosport Central PCN team on a fixed term contract and support the vision to deliver outstanding patient care to approximately 85,500 patients in Gosport. You will be an integral part of the team, working alongside our 5 member practices.

Main duties of the job
The post holder is a pharmacist, who acts within their professional boundaries, working alongside a team of pharmacists in general practice and within the Primary Care Network (PCN). They will improve patients' health outcomes and the efficiency of the primary care team by providing direct, accessible, and timely medicines and expertise including face to face contact with patients. The post holder will be an integral part of the frailty team, working across the PCN, as well as part of a wider pharmacist network and multidisciplinary team. This includes being a conduit of patient medicines information into and out of hospitals. They will work to optimise medication issues to improve patient care and safety and support clinical staff in the management of patients.

Qualifications
Essential

• Master's degree in pharmacy or equivalent
• Registered Pharmacist with GPhC
• Independent prescriber

Desirable

• Peer credentialed by the RPS Faculty (at expert level 2 or greater)
• Diploma or Certificate or MSc in Clinical Pharmacy
