Main duties of the jobThis post supports the CYPCC service assessing children and young people in the community/ home and hospitals to determine eligibility to receive NHS Continuing care in line with the National Framework for Children and Young People's Continuing Care (2016).
The role will ensure that the national assessment tool is adhered to via the required timescales.
The role with include co-ordinating, participating in, and supporting the assessment process with the multidisciplinary team (MDT). The role will be part of the MDT and determine the individual's eligibility in line with the National Framework for Children and Young People's Continuing Care (2016). This will encompass explaining the process to patients and families, health and social care colleagues and care providers. Ensuring that the process is transparent and clearly understood by all parties.
The role will require travel on occasions, to carry out assessments and reviews across Surrey and Sussex. Access to transport is therefore necessary, be it personal or public. All AACC staff are expected to work flexibly, when required, to meet the needs of the service.

Job responsibilities- Supporting the commissioning of health care packages at home for CYPCC eligible children and other community settings to prevent hospital admissions; in addition, they will help support discharge from hospital where ever possible.
- Assessing children and young people in the community/ home and hospitals to determine eligibility to receive NHS Childrens Continuing Care. Utilizing the national assessment tools and adhere to the required timescales to co-ordinate, participate and support the assessment process.
- The role will be part of a multi-agency decision to determine the individuals eligibility in line with the National Framework for Children and Young People's Continuing Care. This will encompass explaining the process to patients and families, health and social care colleagues and care providers. Ensuring that the process is transparent and clearly understood by all parties. Clinical managers for CYPCC will support throughout this process.
- Monitoring and providing assurance on the quality-of-service providers for children and young people, this includes residential placements and short breaks environments.
- In the absence of clinical managers, act in a deputizing role, which would include undertaking duty management and responding to package breakdowns, enquiries for new referrals, checklists and prioritizing workload.
- Assist when required with the monitoring and providing assurance on the quality of service providers for Children and Young Peoples Continuing Care, this includes residential placements and short breaks environments.
- Advising AACC Provider Souring on clinical needs and best practice in relation to services for children with complex health needs and providing clinical input to all parts of the commissioning process.
- To be responsible for planning, managing, co-ordinating and undertaking annual reviews of the Children and young people eligible for CYPCC, within the caseload. Responding to any health needs reported and reviewing the DST and care package accordingly.
- Communicate, with support from the clinical managers, complex information, with empathy and consideration to the children, young people, parents and carers. This may relate to a diagnosis, prognosis and funding issues, which therefore could be highly sensitive or distressing and associated with high degrees of emotion or conflict. This may include end of life discussions.
- Work alongside other agencies, e.g., Local hospices, as well as social services and specialist educational schools, to provide joint care packages where appropriate, ensuring that families receive the most appropriate and equitable service for their child.
- Participate in on going professional development and clinical supervision, including safeguarding supervision.
- Assisting CYPCC team in the delivery of informal educational sessions to varying groups of staff on subjects related to Childrens Continuing care.
- Organise own workload, with oversight from a Clinical Manager.
- Be aware of and follow local Safeguarding policy and follow child protection procedures, providing any reports and attending child protection conferences as necessary.
- Undertake any other duties that may reasonably be required.
Person SpecificationKnowledge, Training and Experience- Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area with up-to-date registration with the relevant regulatory body e.g. Nursing & Midwifery Council (NMC)Registered Nurse or Children's Nurse or Other allied professional with experience of working with children with complex health needs e.g. Registered physiotherapist, occupational therapist or speech and language therapist
- Previous experience of working in a health or care environment (including acute, community, mental health, local authority etc. and evidence of continuing professional development
- Experience and understanding of health and social care environment and roles and responsibilities within it.
- Experience of working in multi- disciplinary teams
- Comprehensive knowledge and understanding of current national and local policy/Framework Guidance relating to NHS National Framework for Children and Young People's Continuing Care and its application in practice
- Knowledge and experience of working with Safeguarding issues and Policy guidance
- Ability to address complex or sensitive issues in an organised and appropriate manner both verbally and in writing at all levels; written documents, spread sheets
- Demonstrates ability to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales
Analytical- Ability to analyse very complex clinical information as part of the assessment process- interpreting both physical and mental health needs, skills required to formulate, develop and ensure implementation of specialist CYPCC care plans to meet identified needs.

- Numerate and able to understand complex financial issues combined with deep analytical skills
- Experience of setting up and implementing internal processes and procedures
- Knowledge of Financial Systems e.g. monitoring budget management, processing invoices and procurement
