Job description

Job summary
We are seeking an enthusiastic and motivated Nursing Associate or (newly qualified Nursing Associate) to join our expanding Single Neighbourhood Team within the PCN. This is an exciting opportunity to work alongside a multidisciplinary team that includes Paramedics, Clinical Pharmacists, Advanced Nurse Practitioners, and Care Coordinators, supporting the delivery of high-quality, patient-centred care across our member practices. You may have just completed your trainee nurse associate training or already be a qualified nurse associate.

Early applications are strongly encouraged, as we reserve the right to close this advert before the closing date.
Interviews will take place on 18th August 2026.

Main duties of the job
The successful candidate will play a key role in managing and supporting our frailty cohort, including palliative care patients, housebound individuals, and those living with frailty. The postholder will contribute to the delivery of Comprehensive Geriatric Assessments (CGAs) and work collaboratively with GPs and the wider multidisciplinary team to coordinate ongoing care and treatment.

The role will also involve delivering a range of nurse-led clinics and undertake housebound and care home visits, including providing vaccinations and, where appropriately trained, procedures such as FeNO testing, ECGs, ear irrigation, and other interventions.

This position offers an excellent opportunity to develop clinical skills, build strong relationships across practices, and make a meaningful contribution to integrated neighbourhood healthcare delivery.

About us
About The Ridge PCN
The Ridge Primary Care Network (PCN) is a semi-rural network established in 2019, serving a population of approximately 36,000 patients across four GP surgeries: Bearsted Medical Practice, Len Valley Practice, Sutton Valence Group Practice, and The Orchard Surgery in Langley. Our PCN has a strong record of collaboration and innovation, successfully delivering a range of services tailored to the needs of our local community. We are supported by an active and engaged Board and benefit from excellent patient participation, helping us to shape and improve services in line with what matters most to our population. A key area of focus for The Ridge PCN is supporting our frail population.
