Job description
We arecurrently seeking a capable, enthusiastic, and committed Care Co-ordinator tojoin our hardworking and friendly administrative team. We require an individual who is keento work in a person-centred way with patients and colleagues to encouragepatient engagement in the quality improvement programmes, appropriatelysupporting those who may experience additional barriers to accessing theservices. This role is also dynamic, multifaceted, and always evolving to provide streamlined administrative support for the practiceteam in delivering the outcomes from the Network Direct Enhanced Service (DES)Agreements. While prior experience with SystmOne and the NHS is advantageous, it is not mandatory. Main duties of the job KEYRESPONSIBILITIES The role will require the strong administrative skills and a capability to quickly learn a good working knowledgeof SystmOne and run SystmOne searches to identifythe patient cohorts, as directed by the clinicians Clinical audit andevaluation relating the Network DES clinical outcomes and quality improvementinitiatives. Administration, monitoring andmaintenance of digital health app dashboards ensuring onward flagging ofanything requiring clinical oversight. Inviting patients to NHS health checks, learning disability annual reviews and mental health reviews. Supporting patients living with mental health is a key focus, and using agreed pathways for triage to practice clinicians and appropriate local services. Supporting the hypertension practice pathways, triage of incoming blood pressure results,onboarding patients to a platform and linking with DigitalTransformation Lead to ensure all requirements and targets of the CCLIP aremet. Checking Shared Care requirements for patientsunder Secondary Care and confirming administrative tasks arecompleted, booking in essential follow-ups or phlebotomy if necessary. Monitoring Diabetes 8 Care Processes, checkingsecondary care blood tests, importing data. Flagging patients requiringadditional support along agreed care pathways About us The Panton Practice has undergone a significant and positive transformation over the past 18 months, guided by a new management structure. Our focus is on enhancing patient experience and improving service efficiency to deliver the highest standard of care. Our multidisciplinary team brings together a wealth of expertise, including GP Partners, Salaried GPs, Nurse Practitioners, Paramedic Practitioners, Practice Nurses, and our dedicated Reception and Administrative staff. Together, we are committed to providing compassionate, responsive, and patient-centred healthcare. Job responsibilities The role will require a good working knowledge of SystmOne in order to be able to set up and run SystmOne searches to identify the patient cohorts, as directed by the clinicians. You will also need to understand how to use the Dorset Intelligence & Insight Service (DiiS). Training will be provided if required. Undertaking non-clinical Cancer Care Reviews with patients over the phone and following any subsequent care pathways. Monitoring C The Signs Dashboard to ensure patients have returned cancer diagnostic testing requirements. Liaising with patients to overcome any barriers to undertaking diagnostic testing. Making Pharmrefer referrals to community pharmacy using the online template. Monitoring and maintaining Spirometry waiting lists and making appointments in Spiro clinics ensuring all clinical exclusion criteria are adhered to. Administration, monitoring and maintenance of digital health app dashboards ensuring onward flagging of anything requiring clinical oversight. Inviting patients for their NHS Health Checks, Dementia reviews, Learning disability reviews and Mental health reviews and ensuring these appointments are booked in a timely manner. Developing an understanding of the Quality Outcome Framework (QOF) and Investment and Impact Fund (IIF) to ensure clinical coding is completed and support the team in achieving targets. Keeping records of the NHS Digital Flag and Reasonable Communication Adjustments required by patients with Learning Disabilities, Autism or other Disabilities. This may include person-centred conversations with the individual to understand their needs. Support the administration of Safeguarding MDTs and incoming correspondence. Monitoring Diabetes 8 Care Processes for the Practice eg checking secondary care blood tests and importing data. Flagging patients requiring additional support along agreed care pathways. Booking structured meds reviews. Working with the First Contact Mental Health Nurses to support referrals to CMHT. This will include creating referral letters and using digital transcribing. Training will be provided. Using clinical letters from secondary care and other health professionals to create coding on patient records. The role will also work in a person-centred way with patients and colleagues to encourage patient engagement in the quality improvement programmes as outlined in the DES and also contribute to a positive and seamless service-user experience, appropriately supporting those who may experience additional barriers to accessing the services. To support the Network and Practice teams with clinical audit and evaluation relating the Network DES clinical outcomes and quality improvement initiatives. * To undertake any other duties or projects commensurate with the nature and grade of this post as required. Person Specification Experience * Proficient in MS office, able to record accurately using a range of electronic systems, * Strong administrative skills, Ability to learn quickly * Experience of using SystmOne, Experience of data analytics and abilities to work with data for audit and evaluation purposes, Experience of working in customer/patient facing roles.
